Hospice Nurse for Huntingdon and Lewistown area:

You'll work alongside other nurses, physicians, and social workers to provide top-notch care to community members in the privacy of their homes. Monday - Friday daylight with on-call rotations and weekend rotations.

Responsibilities:

* Attend compliance training and adhere to the organization standards of conduct, policies, and procedures.
* Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Assigns other nursing personnel, including LPNs and Aides to assist in the appropriate delivery of services as ordered on the plan of care to the patient. Supervises and teaches other nursing personnel including LPNs and Home Health Aides. Identifies, investigates, and recommends solutions to organizational issues. Identifies any deficits or exceptional performance issues of other nursing personnel, LPNs and Aides, and recommends solutions. Proven ability to facilitate change.
* Coordinates case management activities within a multi-disciplinary health care team to insure appropriate delivery of services to patients. Initiates, coordinates, and insures ongoing communication with the physician and other members of the health care team. Attends and participates in patient care conferences, team meetings and other related meetings. Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of UPMC Home Health Care towards patients, visitors, staff, peers, physicians, and other departments within the organization. Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of the services we provide.
* Completes all necessary documentation which may include visit assessment, plan of treatment, OASIS, 485 and Verbal Orders accurately and promptly in electronic documentation system while in the patient's home and within the agency's and regulatory standards. Data syncs (transmits) information on the same day.
* Exercises independent judgement in matters concerning emergent and non-emergent patient care needs and communicates with the physician.
* Participates in on-call coverage rotation for the agency.
* Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, to include complex and/or specialized care.

* HOME HEALTH MENTAL HEALTH PROFESSIONAL STAFF NURSE (Preferred, only required based on department needs)Master?s degree in psychiatric or mental health nursing and must have nursing experience within the last three (3) years in the psychiatric or mental health unit of a hospital, a partial hospitalization program, an outpatient psychiatric clinic, or psychiatric home care. Certification in psychiatric nursing by the American Nurse Association preferred.ORBachelor's degree in nursing and within the last three years, must have one (1) year of nursing experience in the psychiatric or mental health unit of a hospital, a partial hospitalization program, an outpatient psychiatric clinic, or psychiatric home care. Certification in psychiatric nursing by the American Nurse Association preferred.ORDiploma or Associate?s degree in nursing and within the last three years, must have two (2) ears of nursing experience in the psychiatric or mental health unit of a hospital, a partial hospitalization program, an outpatient psychiatric clinic, or psychiatric home care. Certification in psychiatric nursing by the American Nurse Association preferred.,HOME HEALTH INFUSION PROFESSIONAL STAFF NURSE (Preferred, only required based on department needs)One year of acute IV therapy experience preferred.Oncology of Infusion certification preferred.,PROFESSIONAL STAFF NURSE Requirements
* Graduate of an accredited school of professional nursing required. Bachelor's degree preferred.
* UPMC approved certification preferred.
* Six (6) months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care required.
* Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients and their families. Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment. Demonstrates good clinical judgement, critical thinking, and problem-solving abilities. Mobility and visual manual dexterity. Physical stamina for frequent walking, standing, lifting, and positioning of patients.
