PCA Specialist
ACCS Integrated Teams provide clinical interventions and peer and family support to facilitate engagement, support functioning and maximize symptom stabilization and self-management of individuals and to assist individuals in developing skills, establishing natural supports and resources to live successfully in the community.
The PCA Specialist will work in support of all program staff as a member of a Persons’ Team having primary responsibility in delivering personal care with guidance by the team nurse.
The heart of community living service lies in the ability to provide individualized support in a community setting that is based on each person’s strengths, interests and needs. In keeping with the division mission and practice guidelines, staff will strive to genuinely engage Persons served, joining them in exploring their lives, continually promoting recovery, and enabling them to replace formal services with natural supports.
In conjunction with the Integrated Team, the PCA will:
- Provide mobility support and/or assistance with PT exercises
- Assist with maintaining personal hygiene by bathing or showering, toileting, brushing hair and teeth, applying skincare
- Assist individuals with light meal prep, eating
- Record frequency of categories of care
- Support with light housekeeping duties such as laundry, changing bedding, cleaning
- Work closely with Team Nurse, COTA and Elder Care Specialist to coordinate transition to appropriate level of care
Schedule: Full Time, 40 hours
Pay Rate: $22.36/hourly
