Expert knowledge in palliative care, including the pathophysiology of diseases, pain and symptom assessment and management, counseling and communication skills, and advanced care planning
Advance knowledge about caring for individuals with serious, life-threatening illness, as well as those who are imminently dying.
Prepares reports to Medical Director, Palliative and Advance Care and Chief Medical Officer.

- Participate and assist in daily interdisciplinary team meeting (social work, case managers, chaplain)
- Providing palliative education to patients, families and medical staff as requested
- Triaging need-based consults
- Coordinating family meetings and facilitating goals of care conversations
- Completion of all necessary paperwork with patient/family
- Avoiding hospital admissions, on occasion, by being first contact in the emergency department
- Conducts discharge follow-up phone calls
- Reviews all charts with Medical Director
- Coordinates and develops relationships with other sources of referrals and develops good networking with collaborating physician, nursing staff and medical staff.
- Answers and provides community partners with appropriate clinical information via phone, person and other.
- Works to effectively communicate with patients, families and staff/physicians.
- Works collaboratively with MD to provide care to patient population.
- Provides education to the patient/family/nursing staff regarding the disease processes and planned treatment/intervention.
- Provides instruction in the areas of health promotion, activity and medication as appropriate.
- Demonstrates autonomy and ability to work independently and communicate patient issues to physicians.
- Volunteers assistance to others, seeks additional assignments when work is completed, is flexible and willingly accepts additional responsibilities.
- Exhibits sound judgment and makes appropriate decisions in a timely manner.
- Collaborates with the physician as needed concerning pertinent history, physical findings, diagnostic and therapeutic measures initiated.
- Intervenes appropriately in meeting needs of patients and families.
- Collaborates with other health care team members in planning as needed to ensure care.
- Coordinates care with the patient’s primary nurse.
- Documents care provided in the medical record.
- Records changes in patient status, notifications, treatment and response in the medical record.
- Documents plan of care as indicated.
- Writes orders for implementing the plan personally or through collaboration with resident and nursing staff.
- Documents all information in a clear, concise and legible manner.
- Documents information in chronological order at time of occurrence / observation.
- Collaborates with outside agencies to provide services / support for patient.
- Submits appropriate follow-up billing information
Masters in Nursing (MSN) required
Geriatric training required
Licenses and Certifications:
Current APN in NJ
Palliative Care Certified preferred
Knowledge and Skills:
Ability to work independently in emergency situations
Management of palliative and hospice patient
Collaboration with all service line team members
Familiarity with technology used in the critical care and medical surgical units.
Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.

Normal hospital environment with frequent exposure to sick or injured patients. Off site visits necessary to doctors’ offices community events and other practice needs.
Daily potential for exposure to blood and/or body fluids.
