Overview

Job Summary:

The Mobile Assessor is responsible for strengthening relationships with local ER’s and hospitals, while maximizing referral potential from all relevant hospital departments.

The Mobile Assessor will assist local Outreach Coordinator with all hospital referrals and provide ER’s/hospitals with resources needed for patients AAC cannot assist.

Reports to:     Regional Manager Business Development

Responsibilities

Job Duties:

  • Assessor will round as many local hospitals every shift s/he are working to increase presence in the hospitals to increase availability to assist clients.
  • Assessor must build as many relationships as possible with different shifts and departments of hospital including ER, DOU, physical therapy, medical surgical floor, case management department, etc. to maximize referral potential.
  • Assessor must be able to perform quick forms on potential clients via laptop in real time.
  • Assessor must have the clinical knowledge to perform a thorough assessment via laptop in real time.
  • Assessor must call in report to house supervisor at appropriate facility before facilitating a bed to bed transfer.  Including faxing over any documentation the facility may need to complete admission (labs, clearance, etc.).
  • Assessor must have access to insurance verification websites and work closely with insurance teams to be able to facilitate bed to bed transfers in real time.
  • Assessor must call in report to house supervisor at appropriate facility before facilitating a bed to bed transfer.  Including faxing over any documentation the facility may need to complete admission (labs, clearance, etc.).
  • Assessor must have access to insurance verification websites and work closely with insurance teams to be able to facilitate bed to bed transfers in real time.
  • Provide hospitals with quick admissions times.  Clearing beds is usually the first priorities on hospitals, especially in the ER.
  • Provide hospitals resources for clients we can’t accept in real time i.e. calling the resource ourselves for follow up.
  • Work closely with the local psych hospitals to refer them all appropriate clients and keep relations optimal
Qualifications

Job Requirements:

  • Master’s Level Clinical Degree (Social Work, Psychology, etc.), nurse, licensed clinician with MFT, MFT-I, LACD, CADC, CADC-I, LSW, LCSW, PhD, PsyD.
  • Knowledge of SUD and related mental health disorders.
  • Insurance knowledge applicable to AAC’s local facilities.
  • Advance ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings.
  • Advance knowledge of Microsoft Office, including Word, Excel, PowerPoint and Outlook.
  • Ability to perform multi-functional tasks; detail oriented.
  • If in personal recovery, a minimum of 2 years clean and sober.
