Lifestyle Medicine

The position description is a guide to the critical duties and essential functions of the job, not an all-inclusive list of responsibilities, qualifications, physical demands and work environment conditions. TITLE: Lifestyle Medicine Occupational Therapist 

JOB OVERVIEW: The position requires current Occupational Therapy skills to evaluate, plan, implement and/or delegate a comprehensive plan of patient care in 4 areas of service. Provide therapy that meets the needs of the patients with a safe discharge plan including re-assessment/re-evaluation of the patient’s/client’s functional abilities. Ability to assess the impact of simple to complex therapeutic modalities and procedures as outlined in the patient/client plan of care. Excellent communication with all types of patients/clients, caregivers, and healthcare teams is required.

AREA OF ASSIGNMENT: Lifestyle Medicine

HOURS OF WORK: 8 or 10 hour shifts may be available dependent upon patient needs.

RESPONSIBLE TO: Lifestyle Medicine Manager

LELVEL I PREREQUISITES:

  1. Current Washington State OT Interim or Active License.

  2. Ability to evaluate patients, plan and implement therapeutic techniques with written evaluations and treatment plans to reach established goals.

  3. Ability to prioritize tasks for patient care and projects to meet established standards and deadlines.

LELVEL II PREREQUISITES (in addition to requirements for Level I):

  1. Minimum 2 years of current clinical practice and demonstrated leadership ability.

  2. Demonstrated leadership can include specialized patient evaluation and treatment, program evaluation and development, staff/student education and training.

QUALIFICATIONS:

  1. Current BLS certification within 90 days of hire

  2. NBCOT registration within first 12 months of hire

  3. Professional appearance

  4. Effective interpersonal skills

  5. Demonstrate ability to prioritize multiple tasks, set priorities and work accurately and independently

  6. Demonstrate ability to communicate and to work effectively with the physical, emotional, socioeconomical, and developmental abilities and needs of all age groups, cultural and social backgrounds, and other diversity related contributors

UNIQUE PHYSICAL/MENTAL DEMANDS, ENVIRONMENT AND WORKING CONDITIONS:

Follow all Safe Patient Handling policies and procedures and has the ability to recognize own limitations with regards to load handling, regularly seek assistance when demand of transfer or lift exceeds individual physical abilities. Must be able to lift/carry 40 lbs. to complete patient care services. The Occupational Therapist has learned the tasks and skills listed in the Generic and Unique Job Functions section of the job description. They may have specific skills dependent upon specific teams and need continued training to be able to see all types of patients and clients served at VMC. The OT will have mastered skills in multiple service areas and will be expected to provide guidance and leadership skills in program development, quality management, community education, time management, day to day scheduling, student and volunteer supervision and training, staff training and marketing activities and will be seen as a resource to other staff members.

B. Essential Responsibilities and Competencies

  1.  
    1. Perform evaluations that include patient/client functional status appropriate to the patient/client diagnosis needs or condition and pain level if identified.

    2. Collect, analyze and integrate the information in the evaluations from all sources of the healthcare team available including sources such as but not limited to; patient/family interview, patient observation, RN, MD and other discipline specific assessments. Integrate this information to set appropriate individualized plan of care for therapy services.

    3. Elicit information from patient/client or caregiver on a patient’s identified needs and priorities and incorporate this into the assessment and plan of care.

    4. Set specific goals or discharge criteria in the plan of care as determined at the time of the evaluation. Specific goals will assist each patient/client in restoring, improving or maintaining each patient’s/client’s optimal level of functional performance in self-care, self-responsibility, independence and quality of life.

    5. Assess patients in 2 evaluation and treatment areas after initial orientation and mentoring.

    6. Serve as an effective resource to staff in 1 area of practice as validated by peers.

    7. Perform, interpret and document re-evaluations or re-assessments that include the patient's/client's response to care, change in functional performance, completion of goals set at initial evaluation, pain level management as identified by the patient and that is appropriate to their individualized plan of care. (Determine the need for further treatment, discharge or referral to appropriate services.)

    8. Attend assigned team conferences/floor rounds and provide appropriate patient care information and alter treatment plan as needed by incorporating information obtained at the team conferences.

  2. Evaluate and recommend optimal level of care setting for patient and/or discharge as appropriate to home programs or community resources, home safety equipment, etc.

  3. Provide appropriate information to subsequent providers related to the patient’s/client’s plan of care and services when a patient moves to different services.

  4. Provide patient/client education that will assist them in meeting their goals and increase their functional skills and improve their quality of life and health.

  5. Delegate patient care appropriately with clear communication to lead to successful patient outcomes.

  6. Assist or provide 1 community education program per year.

  7. Provide 1 staff educational activity per year.

  8. Complete documentation - same day for inpatient services and within 3 business days or before next visit for outpatient services.

  9. Interact appropriately with equipment vendors and gather information on new products to enhance patient care. Provide information to the Clinical Lead, Supervisor, Manager or Lifestyle Medicine Director as applicable.

  10. Complete certification for care in a timely manner to allow for continuity of care for outpatients.

  11. Initiate and maintain current information on the progress summary list as appropriate for each outpatient.

  12. Follow policies and procedures for appropriate billing in a timely manner for patient care. Understand rules, laws and regulations pertaining to patient billing and documentation for quality patient care.

  13. Utilize technology appropriately for clinical work, patient care and quality standards. Technology can include but is not limited to: VMC software programs, email, Internet, voice mail, fax, etc. Create an efficient environment for teamwork and quality patient care.

  14. Demonstrates knowledge of life safety policies and procedures in assigned area/location of practice.

  15. Provide feedback to team or Director on program evaluations and protocol development for improvement in specific programs or overall service delivery.

  16. Network with a minimum of 2-4 professional community resources to assure community standards are met.

  17. Review, revise or create a minimum of 2 protocols/procedures per year to improve patient care activities.

  18. Network with a minimum of 2-5 professional community resources to assure community standards are met. This may include providing, proofing or creating patient/community educational materials or programs to enhance community health and/or patient care delivery.

  19. Understand values and incorporate elements of evidence-based practice into clinical activities. Share this with others and show evidence of growth in practice area.

  20. Participate in the development and implementation of new programs and/or program changes so that a specific service or program can expand and better serve the community.

  21. Participate in 1 marketing activity per year to promote Lifestyle Medicine Services clinics or programs.

  22. Participate in ongoing identification of risks for department or team and provide feedback regarding the development of strategies for preparedness.

  23. Fulfill clinical/non-clinical role, as assigned during an emergency. Continue to provide appropriate care, treatment and services to our patients during an emergency.

  24. Provide feedback to team on evaluation of emergency operations for department/team and assist to return department to normal operations as assigned.

  25. Provide observation student supervision experiences after orientation completion. Additional student responsibilities can be provided as coordinated by the Director or designee.

  26. Comply with the professional ethics and code of conduct in professional organization, e.g., American Occupational Therapy Association.

  27. Once certification is obtained, maintain current NBCOT certification.
