Job Description


We have an immediate opening as an Onsite Telephonic LMSW / LCSW / Registered Nurse / RN Case Manager for our growing Case Management team!

As the Onsite Telephonic LMSW / LCSW / Registered Nurse / RN  Case Manager will focus on assessing, plans, implementing, coordinating, monitoring, and evaluating care for their behavioral health members. 

DAILY RESPONSIBILITIES FOR ONSITE TELEPHONIC / LCSW / LMSW  REGISTERED NURSE / RN CASE MANAGER :

  • Promote quality and cost-effective outcomes
  • Assesses, plans, implements, coordinates, monitors, and evaluates the care plan
  • Analyze medical records

REQUIREMENTS FOR ONSITE TELEPHONIC LCSW / LMSW / REGISTERED NURSE / RN CASE MANAGER :

  • 2+ years’ experience in Case Management / CM and Utilization Management / UM / UR
  • Health Plan / Managed Care experience
  • Behavioral Health experience 

SALARY FOR ONSITE TELEPHONIC LCSW / LMSW / REGISTERED NURSE / RN CASE MANAGER :

Starting at $70K+ with bonus
