The Integrated Ambulatory Services (IAS) Advanced Practice Navigator incorporates the role of advanced practice providers into clinical practice, education, research and leadership in addressing the health care needs of all customers in Integrated Ambulatory Services. This includes but is not limited to the breast care program, lung cancer screening program, diagnostic imaging, screening services, infusion services, and moderate sedation and pain management. This position will serve as the conduit for services to provide a patient with the experience of integrated care. The Navigator facilitates patients’ interactions throughout the care continuum and among service lines including the physician, physician offices, outpatient providers, radiologists, oncologists, tertiary care centers, other specialties such as surgery and internal departments of the healthcare delivery team. The Navigator coordinates care by guiding the patient through the care process and connecting the patient with appropriate services.

Minimum Education

  • Master’s Degree in Nursing (NP) or Graduate of an American Medical Association-approved Physician Assistant educational program (PA)

Minimum Work Experience

  • 5 years of clinical experience as an Advanced Practice Provider
  • Experience in Ambulatory Care as a Navigator or equivalent role preferred
  • Experience in Interventional Radiology or Oncology preferred

Required Licenses/Certifications

  • For NP: Registration with the Vermont State Education Department as a Nurse Practitioner and Certified as a Nurse Practitioner by the American Nurses Credentialing Center or equivalent certifying bodies
  • For PA:  Registration with the Vermont State Board of Medical Practice as a Physician’s Assistant and certified as a Physician Assistant by the National Commission on the Certification of Physician Assistants
  • ACLS and PALS or willingness to obtain within 6 months

  • Knowledge of quality improvement methods, data analysis methods
  • Demonstrated strong written and verbal communication skills
  • Demonstrated strong knowledge of patient clinical service delivery
  • Excellent diplomacy, problem-solving, conflict management, team building, and collaboration skills
  • Excellent organizational and time management and self-motivation skills
  • Strong Microsoft Windows desktop application and navigation skills
  • Advanced EMR skills
