The RN Medical Case Manager (RN MCM) blends the mission of AHF with Medical Case Management, utilizing strong nursing clinical skills and an understanding of the Ryan White Care Act Program within their specific area. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that patients receive the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly within the Healthcare Center, you will complete Health Risk/Needs Assessments, develop Action/Service/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications. For Ryan White contracts that combine psychosocial needs within the RN MCM, you will also provide services addressing mental health needs and basic needs. By developing relationships with programs and agencies serving those needs, you are able to facilitate referrals, and provide support and assistance for your patients who lack those services. In this way you will support the patient's ability to better understand their health status so that they can work to achieve their healthcare goals.

By utilizing core principles of nursing, you will meet our members where they are, identify strengths and opportunities for growth and assist them with realistic goal setting. Advocating for your patient, you will bring the essence of the Ryan White Care Act - the safety net for the uninsured HIV/AIDS patients - to our most vulnerable population. By establishing Health Risk/Needs Assessments, Service/Action/Care Plans, referrals and support will directly contribute to AHF's success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

The RN MCM is often the liaison between provider and patient and facilitates the shift from the exam room to implementing the optimum plan of care, providing brief interventions focused on achievable goals. The patient and the RN MCM work together each step of the way towards viral suppression, better health outcomes and quality of life.

Education and/or ExperienceRN with a minimum of Associate of Nursing degree preferred or Earned Bachelor degree from an accredited institution with a minimum of three years Case Management experience required. In addition an extensive:

* Knowledge of community resources
* Knowledge of target population
* Knowledge of HIV disease and treatment
* Competence in Cultural and linguistic Diversity.
* Experience in care

Other Skills & Abilities/QualificationsMinimum one year managed care and case management experience.Excellent written and verbal communication skills.Excellent organizational skills.Knowledge of ICD-9 CodingClinical experience in the care of people with HIV/AIDS.Must be able to travel 50% of the time.Spanish-language skills preferred.

Certificates, Licenses and RegistrationsRegistered Nurse licensed by the State of Florida if appropriate. Certification in HIV/AIDS nursing (ACRN) within 24 months of employment. Certified Case Management (CCM) preferred. Valid Florida Driver's License with proof of automobile liability insurance.
