FAMILY MEDICINE NURSE PRACTITIONER

Sinai Health System Chicago, IL Full-Time

General Summary: The Nurse Practitioner, in collaboration with the physician provides medical care and treatments for patients under a Sinai Physician's care

  • The Nurse Practitioner will work within the collaborative practice agreement as agreed by the NP and the designated physician(s)

  • The NP is responsible for assessment and treatment

  • The NP will participate in Quality Assurance / Quality Improvement

  • The NP will emphasize health promotion, maintenance, disease prevention, diagnosis and management of acute and chronic diseases

  • Reports to : Family Practice Chairman

  • Conducts new patient evaluations and manages the care of established patients in clinic

  • Assess patient status through history taking, physical assessment, laboratory and diagnostic interpretation

  • Instructs patient and / or family on disease process and care plan

  • Provides general and preventive care

  • Provides counseling in adapting to and coping with changing health status as appropriate to patient and/or family

  • Ensure continuity of care by coordinating the patient health care and psychosocial needs

  • Establish priorities and makes ethically sound decisions to ensure safe patient care

  • Provides patient education

Education:

  • Graduate of an Accredited Nurse Practitioner program - Master's Degree Nurse Practitioner

Experience:

  • 2-3 Years of in patient & out patient clinical based practice experience preferred

  • New Grads accepted

Knowledge, Skills and Abilities:

  • Excellent verbal, writing and interpersonal communication skills

  • Effective leadership and organization skills

Required License , Certificates, Registrations:

  • Current State Licensure of Illinois Nursing License

  • Current Advanced Practice License

  • BLS CPR
