The Certified Nursing Assistant is responsible for providing direction and assistance with the activities of daily living and personal care necessary for each resident to remain as independent as possible.
Follows established standards of nursing practice and implements facility policies and procedures.
Listens to resident and family concerns and initiates corrective actions and communicates concerns and outcomes to the Nurse.
Interacts with customers and their families to build relationships, collect information that assists in meeting their personal needs, and ensure their satisfaction with the personal care services provided.
Provides support and encouragement to all residents to remain as independent as possible.
Provides residents with assistance in activities of daily living as documented in the resident’s care plans.
Assures residents are given adequate assistance with personal care and oral hygiene.
Assists with ambulation, transferring or transporting of residents, (including pushing residents in wheelchairs) to the dining rooms, activities and/or various areas of Prairieview as necessary.
Communicates any change in resident condition to the Nurse immediately.
Assists with serving meals and beverages, supervising dining rooms during meal times to ensure resident safety.
Reinforces principles of good nutrition and assist residents with menu choices as necessary
Encourages good nutrition and hydration.
Delivers personal mail to each resident room as assigned.
Observes all facility safety policies and procedures.
Identifies safety hazards and emergency situations and resolves the issue immediately, reporting issue and outcome to the Nurse.
Observes infection control procedures.
Completes resident rounds as assigned, reporting any resident care issues to the Nurse on duty.
Provides respect, privacy and dignity to all Prairieview residents; does not enter any resident room inappropriately.
Assists with orientation of new residents to the facility.
Investigates all door alarms immediately, affirming the cause of the alarm prior to clearing and resetting of the alarm.
Responds in a timely manner to the emergency call system and portable paging system.
Assures that resident call lights are within reach of resident.
Maintains accurate records of each resident’s daily care on the appropriate documents, and in accordance with state and federal regulations.
Maintains confidentiality of resident records and facility information.
Follows resident rights protocol at all times.
Reports any possible cases of Resident Abuse immediately to the Nurse on duty.
Attends all in service classes as assigned.
Participates in the interdisciplinary team and other facility meetings as assigned.
Participates in the development of an individualized plan of care for each resident.
Reviews care plans daily and performs nursing care as outlined.
Completes assignments timely, completely and accurately.
Consistently works cooperatively with residents, Nurses, other department personnel, physicians, consultant personnel and ancillary service providers.
Works collaboratively with all team members and takes whatever action is necessary to provide the highest level of service to all Members of The Garlands.
Performs other related duties as directed by the Nurse on duty.
Comes to work in a clean neat uniform, and consistently presents a professional appearance.
Comes to work as scheduled and consistently demonstrates dependability and punctuality.
CUSTOMER SERVICE & COMMUNICATION
Demonstrates excellent listening skills and communicates effectively while conveying patience and understanding.
Writes clearly and concisely.
Responds to customers by promptly researching various options and following through with necessary actions in a timely and effective manner.
Treats all customers with dignity and respect and establishes a trusting relationship with them, while maintaining confidences.
Demonstrates knowledge of The Garlands’ entire facility and directs customers appropriately, according to their needs.
Completes Incident Reports accurately and timely.
Completes Concern Reports accurately and timely.
Completes all appropriate documentation accurately and timely.
Communicates any allegation of resident abuse immediately to Nurse.
Knowledge, Skills, & Abilities:
Experience working in assisted living, home health or long-term care environment preferred.
High school diploma or its equivalent.
Nursing Assistant certification.
Must possess, or be able to obtain current CPR certification.
Must have an adequate understanding of English and be able to speak and write it well.
Must be able to work independently, as well as a team member, with a positive, pro-active attitude.
Is flexible and adapts to change and new tasks.
Follows all policies and procedures of Prairieview at The Garlands.
Must be familiar with the Rights of the Resident, the definition of Resident Abuse, and the reporting protocol of possible Resident Abuse.
Must be a flexible, highly customer service and people oriented individual who demonstrates compassion, patience and understanding, and is willing to do whatever it takes to provide the highest level of service.
Must be a team-oriented individual who enjoys working collaboratively with others, creating a positive and uplifting environment, and communicating very effectively at all levels.
Must be able to manually lift and carry up to 50 lbs. frequently.
Must be able to assist in transferring residents who weigh in excess of 50 lbs. occasionally.
Must be able to tolerate long periods of walking, bending and standing.
Must be able to push customers in wheelchairs frequently for brief periods of time.
