Responsibilities

Job Summary

  • This job is responsible for the coordination of Heart Failure (HF) initiatives regionally across CHI Franciscan Health (CHI-FH) and for related community outreach, including developing, implementing, managing and evaluating a comprehensive HF program in keeping with professional standards, applicable regulatory requirements and business/patient care objectives of the organization.
  • Work is focused on establishing and maintaining a comprehensive and seamless inter-disciplinary continuum of prompt and high quality patient care to meet the unique and specialized needs of heart failure patients and to foster optimal clinical outcomes.
  • An incumbent works collaboratively with a multi-disciplinary team on program development/implementation efforts, and serves as a clinical expert to healthcare providers, hospital staff, patients, families and the community at large with respect to all aspects of heart failure disease management and care.
  • Work includes developing and delivering education to patients with emphasis on symptom recognition and risk factor modification, as well as ongoing training/support to healthcare providers in general heart failure care and the latest treatment protocols/options.

Essential Job Functions

  • Leads and/or coordinates the development, implementation and organization of CHI-FH HF Team Algorithms/Protocols/Clinical Pathway/Order Sets in accordance with established policies, procedures, standards and regulatory requirements.
  • Develops (in conjunction with interdisciplinary team) plans and procedures to foster HF prevention/treatment/rehabilitation programs, address efficacy/cost effectiveness of services provided, enhance customer service and generate increased volume/revenue.
  • Conducts ongoing review and monitoring of HF care-related documentation to ensure continued adherence to standardized HF orders, nursing pathway and protocols and that care given was appropriate based on established regulatory requirements/guidelines; identifies issues, determines causes and provides education/guidance to preclude the occurrence of future issues.
  • Assesses and reports on program including costs, patient/clinical outcomes and patient satisfaction.
  • Keeps abreast of current and emerging technological developments of interest to the organization and regulatory requirements with potential for impacting existing programs; assesses applicability to current initiatives/objectives, identifying opportunities to enhance current processes/outcomes; develops recommendations as appropriate.
  • Assesses and refines the CHI-FH strategy for HF disease management programs based on national trends that affect the provision of service, expectations of quality, and financial performance.
  • Explores the capacity to market services to meet the needs of the heart failure patient population.


Qualifications

Education/Work Experience 

  • Bachelor’s degree in nursing, and three years of recent related work experience that demonstrates the attainment of the requisite job knowledge, skills/abilities, preferably in an acute care hospital in Cardiac, Emergency, Trauma or Critical Care nursing.
  • Master’s degree in nursing is preferred.

Licensure/Certification

  • Current licensure as a Registered Nurse issued by the Washington State Board of Nursing.
  • Current Basic Life Support (BLS) for Healthcare Providers through the American Heart Association.
  • CHFN, PCCN, CCRN and/or Case Management Certification is preferred.
