Case Manager, Families of Promise Program, Lake County
This program serves youth and their current caregivers due to the incarceration of the youth's parent(s).It is a prevention and supplemental behavioral health program that collaborates with a multitude of community resources with the aim of providing stability and improving functioning of family members. Services also provide the caregivers opportunities to enhance their parenting strategies.
Responsibilities:
- Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
- Work with children, caregivers, and parents to address unmet needs and support them as they navigate through their barriers.
- Provide direct service either in the office, via telehealth or in the field/homes.
- Initiate and coordinate interventions, review and monitor progress, and reassess needs to ensure successful outcomes.
- Assist clients in developing formal and informal community supports, while partnering with local agencies and resources.
- Provide advocacy on behalf of clients.
- Prepare and complete all required case recordings in an electronic medical record, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
- Attend clinical and administrative staff meetings, and multi-discipline team meetings as required.
- Review treatment plans and progress of cases with supervisor for continued support, advice and approval.
- Perform other duties as necessary to accomplish objectives.
