A large national home care provider is seeking a full time Licensed Practical Nurse to provide skilled nursing care to clients in the home as delegated by, and under the supervision of, the Registered Nurse.
Graduate of an accredited school of professional nursing.
Current State License as a Licensed Practical Nurse.
Preferably 1 year of experience in an acute care setting, or equivalent experience.
Effective written and verbal communication skills. Good interpersonal communication skills.
Reliable transportation required.
Sit, stand, bend, lift, and move intermittently. (Must be able to lift 80-100lbs).
Read, speak and write English in order to fulfill job functions in an understandable way.
Participate in implementation of the Plan of Care to ensure quality and continuity of care.
Verify the Plan of Care prior to each visit and provide care according to delegation by the RN, physician’s orders, assessment data, and established standards and guidelines.
Communicate significant findings, problems and changes, in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care.
Assist the patient in learning appropriate self-care techniques.
Suggest revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager.
Remain available to the RN Case Manager during normal business office hours for advice, assistance, and instructions.
Document medication regimen and update medication profile according to established procedures.
Treat patients and caregivers in the highest and most effective manner to improve patient outcomes.
Ensure all nursing notes, and supply logs/visit logs are accurate and turned into office according to office policy.
Demonstrate knowledge of current status of requirements of third party payor, by identifying and reporting potential payment/coverage problems to appropriate supervisor.
Responsible for protecting patient and staff privacy and confidentiality at all times pursuant to the HIPAA Privacy Final Rule.
Maintain availability by phone as directed by company policy.
Report to work and patient homes promptly as directed.
Handle the personal property of patients and their families with care.
Follow all infection control guidelines and standards per Agency policy.
Perform other duties as assigned by the Supervisor.
