This is a work at home position with 60-70% of the time in the field, both homes and hospital setting. We are hiring in these counties: Kings, Suffolk, Nassau, Onondaga and Queens
The overall purpose of the Pediatrics Behavioral Care Advocate is to improve the enrollee's ability to remain stable in the community and out of the hospital. The role is to engage the person in the treatment process and then assist them to access the appropriate community services so they can pursue their treatment goals in the community and avoid hospitalization.  This position will be making field visits going into member homes, hospitals, clinics and other facilities.
The Pediatrics Behavioral Care Advocate position is also designed to be a direct clinical liaison between health plan staff and the clinical network.  The program fosters a collegial and mutually beneficial relationship between the providers and payers of medical and behavioral health services. 
If you are located near these counties: Kings, Suffolk, Nassau, Queens, New York, you will have the flexibility to telecommute* as you take on some tough challenges.  
Primary Responsibilities:
  • Identifies needed services and matches with available services in area
  • Assists enrollees with appointments
  • Provides subsequent member follow-up as determined by individual member needs
  • Works with enrollees to identify gaps in care or obstacles to care and problem solves for successful connection to needed services
  • Reviews IP Census each day to determine which enrollees on the census are re-admits within the last 12 months
  • May go to facility to meet with treatment team to develop a successful aftercare plan
  • Manages the aftercare follow-up of identified high risk enrollees (manages follow-up calls/outreach and documentation)
  • Meets with community providers/facilities as company liaison to foster good rapport and relationships and to identify new programs
  • Works with CNS to identify new facilities and programs that provide services to targeted populations
You'll find the pace fast and the challenges are ongoing. You'll be expected to document and achieve measurable results. You'll also need to think and act decisively while working with a diverse member population.

Required Qualifications:
  • Must have one of the following:  Master's degree in Behavioral Health, Bachelor's degree in Nursing, Associate's degree in Nursing with ten years of Behavioral Health experience, or a Doctorate-degreed Psychologist
  • Must have an active, unrestricted State of New York professional license in at least one of the following:
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Professional Clinical Counselor (LPCC) or Licensed Mental Health Counselor (LMHC)
    • Independent Marriage & Family Therapist (LMFT)
    • Licensed Masters Social Worker (LMSW)
    • Licensed Psychologist (PhD)
    • Registered Nurse (RN), Pediatric Behavior Health Specialty
  • 2+ years' experience in a pediatric behavioral health environment
  • Proficient computer skills and good working knowledge of Microsoft Word
  • Reliable transportation and ability to travel around designated territory in New York
Preferred Qualifications:
  • Knowledge of Level of Care Guidelines (i.e., Milliman, Interqual, etc.)
  • Managed Care Experience
  • Experience in discharge planning and/or chart review
  • Experience in inpatient/acute setting or outpatient setting
  • Bilingual language skills
  • Field based experience
  • Case management experience
