Our new free-standing Memory Care Community is now open and is staffed as a true Household Model with the focus on self-led universal workers who form deep relationships with the residents they serve. The three-story community will feature three 16-residence households (two assisted living and one skilled nursing), each with its own shared community living and its own therapy space, salon and chapel.

We are currently seeking a full-time Universal Household Life Enhancement Guide. Mon-Fri from 8:30am-5pm, with alternating weekends off.
KANSAS CNA PREFERRED

EDUCATION AND EXPERIENCE
• Active certification as Activity Director in in the state of Kansas, preferred
• Active certification as Certified Nursing Assistant in the state of Kansas
• Current CPR/AED certificate of completion
• Previous experience in memory support long term care setting or similar therapeutic recreation programs preferred.

PRIMARY/ESSENTIAL DUTIES
The Household Life Enhancement Guide must be capable of performing the following primary and essential duties:
• Household Leadership: Maintains and encourages each household’s ability to plan and carry out personal interests and social activities for residents.
• Programming: Plans and carries out opportunities according to resident preferences.
• Care Planning: Facilitates household councils; interviews residents and others to assess and evaluate residents’ recreational needs, interests and preferences. Works with resident and others to develop a plan of care.
• Special Events: Facilitates the organizing and implementing of special events.
• Materials: Develops and maintains a library of suggestions and supplies.
• Observation: Observes residents closely to identify changing needs and conditions at first indication. Assists licensed team members in responding to code situations.
• Clinical Records: Completes activity sections of comprehensive assessment tool (MDS). Develops and reviews care plans and signs off on them as required.
• Safety: Follows proper infection control practices.
• Continuing Education: Attends and completes in-service education programs as assigned.

*Retention Bonus is paid out in increments of 30, 60, and 90 days and the final payout is given on your 1 year anniversary.
