Position Summary: Provides professional skilled nursing services, with a primary focus on prevention, education and treatment of infectious disease, to individuals/families in the home or in place of residence as appropriate and in accordance with standards of professional nursing practice, as well as VNA PHILADELPHIA policy, procedures and standards.

Essential Functions:

• Demonstrates knowledge of, and applies, professional nursing principles, practices and techniques to Home Health Nursing assignments and patient care with a focus, but not limited to infectious disease management.
• Plans and manages assigned caseload in collaboration with Patient Care Manager; assesses schedule daily and determines priorities on the basis of patients' levels of health care needs and payor-defined protocols.
• Assures conformity to performance standards including outcomes, quality and quantity, as defined by VNA PHILADELPHIA and other pertinent regulatory or payor entities.
• Compliance with, and support for, VNA PHILADELPHIA policies and practices throughout operations and delivery of care, as aligned with VNA PHILADELPHIA Mission and Values.

Knowledge and Experience:

• Current Pennsylvania RN License or ability to acquire immediately.
• Current Driver's License and auto insurance.
• Knowledge of case management standards and protocols for delivery of care and other needed services

Education: Accredited RN diploma or RN degree program.
