Key Responsibilities:

  • Responsible for coordination an ongoing program of diversified activities designed to meet, in accordance with the comprehensive assessment, the interest of each resident in the Long Term Care Facility.
  • Responsible for the provision of group, individual and bedside activities in accordance with individual preferences and the physical, mental, and psycho-social well-being of each resident.
  • Responsible for organizing and supervising volunteers and students whom assist with activities.
  • Responsible for bulletin board displays, holiday and special events decorations, and other visual displays associated with activities and recognition events for residents.
  • Responsible for assisting the transportation of resident on the LTC van to scheduled doctor’s appointments or other approved scheduled outings.
  • Responsible for assisting LTC Social Worker with the following as needed:
    • Assisting with new admissions comprehensive social history and assessment that is conducted by the LTC social worker.
    • Regular visits with residents and/or family to assess phycho-social functioning, personal needs and discharge plans
    • Assisting residents with arrangement to keep in touch with their communities, cultural heritage and former lifestyle
    • Assisting with religious needs
    • Helping the resident maintain contact with family and friends
    • Pre-admission screening and assisting with the Completion of PASSAR Medicaid Pre-Admission Screening on all residents
    • Assisting with Resident Council Meetings and Quarterly Family Council Meetings.

Marginal Responsibilities:

  • Complete and submit to Nursing Home Administrator the monthly Census Statistics and monthly Death report
  • Assist in the transmission of the MDS+ Assessment, per regulatory requirements, as assigned
  • Assist with the special projects requiring typing reports, making copies, etc. as assigned.

Language Skills:

  • Good communication skills, both orally and written
  • Ability to interact well with residents (individually and in groups) families, staff and volunteers is essential.
  • Ability to read, analyze and interpret professional journals and periodicals.
  • Ability to write reports and correspondence, as well as present information and respond to questions from groups of managers, employees, residents and families.

Mathematical Skills:

  • Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percentages is needed.

Reasoning Ability:

  • Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to analyze, interpret and comply with all professional, State, Federal, and JCAHO standards and regulations concerning activities and social services.

Minimum Education

  • Completion of an activity training course approved by the State of MS; or
  • Qualified Occupational Therapist or Occupational Therapy Assistant; or
  • Social Work Degree who has 2 years’ experience in a social or recreational program within the last 5 years.
