POSITION SUMMARY: The Assistant Branch Director/Packet Review & Quality Improvement position is responsible for the review and approval of all patient information that is provided by the Licensed Professional during a Start of Care, Re-certification, Resumption of Care, or Evaluation visit. This positon promotes quality patient care; ensures stable relationships with referral sources, patients, caregivers, and employees, and is responsible for assisting with training of Branch staff.



  1. Completes review of evaluation packets, OASIS and 485s.   Assures consistency with the comprehensive assessment and the 485.  Reviews both the data submitted from the field staff to ensure accuracy and follows up on any documentation that requires correction.

  2. Ensures that service utilization correlates with assessment data.

  3. Processes the OASIS and 485, and verifies the correct start of care date and episode date range. 

  4. Processes any unlisted supplies, unlisted medications, activity, functional limitations, allergies, etc. that appear on the action screen to assure completion of the 485.

  5. Ensures that appropriate Care Types and NDPs are selected based on client's medical condition and staff's assessment data.

  6. Reviews ROC and Discharge Assessment Data to ensure accuracy and follows up on any documentation that requires correction.

  7. Processes the ROC and Discharge OASIS and orders.

  8. Assures that the client's plan of care is completed and reassessed by the appropriate health care professional when there is a significant health status change in the client's condition, at the physician's request and after hospital discharge.  Assure appropriate documentation is completed for all patients transferred to an inpatient facility.

  9. Assists the Branch Director with conducting OASIS and documentation portions of the Job Specific Orientation as requested. 

  10. Assists the Branch Director with identification, then additional training for employees on OASIS documentation.  Keeps Branch Director appraised of any additional training needs for staff.

  11. May be required to perform patient visits.

  12. May be required to participate in on-call rotation.



  1. Reviews Infection Control reports for all clients.  Provides / documents all follow up related to Infection Control Reports (both client and employee).

  2. Reviews Occurrence Reports.  Provides/documents all follow up related to Occurrences. 

  3. Functions in the capacity of staff nurse when necessary and participate in community awareness programs as necessary.

  4. Takes administrative on-call when required as a support to the on-call nurse and to provide software management related to processing intakes, reviewing evaluation packets, and scheduling, moving, reassigning visits to ensure the smooth function of the Branch during off hours.

  5. Follows up on OASIS Assessments that cannot be processed due to Licensed Professional document deficiencies.

  6. Reviews Infection Control reports for all clients.  Provides / documents all follow up related to Infection Control Reports (both client and employee).

  7. Reviews Occurrence Reports.  Provides / documents all follow up related to Occurrences. 

  8. Functions in the capacity of staff nurse when necessary and participate in community awareness programs as necessary.

  9. Adheres to and participates in the Branch's mandatory HIPAA / Privacy Program and Employee Compliance Program.

  10. Reads and adheres to Branch policies and procedures and follows Employee Handbook guidelines.

Completes all other duties as assigned.

  1. Must be a graduate of an accredited school of nursing.

  2. Must be licensed as a Registered Nurse in the state of practice. Bachelor's Degree preferred.

  3. Home care management experience required.

  4. Must have a minimum of one year nursing experience within the previous 36 months.

  5. Must possess knowledge of the Medicare guidelines governing home health and hospice agencies.

  6. Prior packet review/QI experience preferred. Coding certification preferred.

  7. Must have exceptional customer service skills and be able to communicate well with referral sources, patients, family members, and branch employees.

  8. Must have/maintain a valid driver's license and auto liability insurance.

Must be flexible with work schedule.
