The Adult Home Health team delivers care to patients in the community, providing a wide range of skilled nursing care. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain in living in the setting of their choice, their home.

The Community Health Nurse (CHN) RN will be responsible for the delivery of a full range of skilled nursing care to individual clients in their homes and other community settings. Care is family centered, age specific and culturally appropriate. This role supports patients to live in the setting of their choice, and is integral in keeping individuals in their homes. The CHN develops meaningful relationships with their patients and provides hands on care.

The Community Health Nurse II (CHN) provides nursing care to clients utilizing the Nursing Process and within the framework of ANA Standards of Practice for Home Health Nursing, and Specialty Specific Standards of Patient Care. (CWOCN, APA, etc.) The CHN II assumes leadership responsibilities, including delegation of work to other employees.

Qualifications

  • ASN required
  • Greater than one year of home or public health nursing experience preferred, or a combination of education and experience from which comparable knowledge and skills are acquired
  • Current RN licensure or compact licensure recognized by the State of Vermont required.
  • Previous home care desired but not required. We are willing to train and develop a candidate who demonstrates a willingness and desire to learn skilled nursing.
