A healthcare organization in Brooklyn is actively seeking a compassionate and dynamic professional to join their staff as their new Case Manager. In this role, the Case Manager will provide case management services for a caseload of 22 clients and their family members.  


The Case Manager will:
  • Ensure that each client is receiving appropriate and effective case management services
  • Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals
  • Conduct face to face contacts (home visits/office visits) as specified in the service plan(s)
  • Provide assistance in accessing primary medical care
  • Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs
  • Provide crisis intervention to clients and their families as needed
  • Maintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs
  • Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
  • Bachelor's degree
  • 1-2+ years of field work experience HRA/HASA direct work experience
  • Individual, family, and group counseling experience, particularly with HIV+/AIDS affected populations and/or substance abuse individuals
  • Extensive knowledge of case management principles and practices
  • Good initiative and learning ability of HRA-HASA Scattered Site Program Desk Guide and the HIV/AIDS Program, Office of Mental Health regulations
  • Highly organized
