A healthcare organization in Brooklyn is actively seeking a compassionate and dynamic professional to join their staff as their new Case Manager. In this role, the Case Manager will provide case management services for a caseload of 22 clients and their family members.

- Ensure that each client is receiving appropriate and effective case management services
- Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals
- Conduct face to face contacts (home visits/office visits) as specified in the service plan(s)
- Provide assistance in accessing primary medical care
- Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs
- Provide crisis intervention to clients and their families as needed
- Maintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs
- Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
- Bachelor's degree
- 1-2+ years of field work experience HRA/HASA direct work experience
- Individual, family, and group counseling experience, particularly with HIV+/AIDS affected populations and/or substance abuse individuals
- Extensive knowledge of case management principles and practices
- Good initiative and learning ability of HRA-HASA Scattered Site Program Desk Guide and the HIV/AIDS Program, Office of Mental Health regulations
- Highly organized
- Experience in Scattered Sites Programs
- Bilingual, English and Spanish
