JobSummary: Brockton Home HealthAgency is looking for a Lead Home Health Clinical Manager, inspire and motivateclinical manager to run our Mass Health/Medicaid, Medicare and AFC program.

As a Clinical Manager your day may look like this!
•The Clinical Manager coordinatesand oversees direct care patient services provided by clinical personnel
•Responsible for and oversees thedelivery of care to Brockton HHA patients
•Assisted and assign all referralsto the proper discipline and initial visits are done timely.
•Accountable to ensure patientsmeet admission criteria and make the decision to admit patients to service
•Assist and assigns appropriateclinicians to a case, as needed
•Assist with clinical questionsfrom insurance providers
•Manage a team of Clinicians(PT's, RN's and LPN's)
•Coordinates communication betweenBrockton HHA and insurance providers
•Provides direct patient care onan infrequent basis and only in times of emergency
•Maintains relationships withreferral/community sources
•Assist with PA review beforesubmit to insurance providers
•Communicate with clients and their families to establishcare plans and ensure all needs are met.
•Coordinate with community resources and healthcareprofessionals to enhance Brocktonhha's brand awareness, attending meetings andconnecting with potential referral sources.

Qualifications
•Registered Nurse (RN) with 5 or more years of experience asan RN.

• Supervisory experience is preferred.
• Passionate about mentoring/ educating others.
• Excellent communication, time management, and organizational skills.
• Ability to work both independently and in a team.
