We are seeking a compassionate MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process, ensuring compliance with CMS guidelines and facilitating the development of interdisciplinary care plans.
Key Responsibilities:
- Maintain MDS status
- Schedule assessments
- Monitor patient conditions
- Coordinate care plans
