The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities.  RHD's multi-faceted programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse and homelessness.

RHD Delaware's Intensive Case Management (ICM ) team provides non-traditional community based mental health treatment to promote the recovery and community integration of adults with serious mental illnesses and co-occurring substance use disorders living in the community to reduce use of ERs and inpatient hospital stays.  The ICM Teams serves all of Sussex County and lower Kent County.

The Recovery Coach, who is an integral part of the mulit-disciplinary team, self-identifies as an individual with an addiction who is currently or formerly a recipient of addiction services.  This is a part-time, field-based position and requires at least 75% of services to be provided to participants in their homes, shelters, places of employment, vocational and education settings and other community settings.

  • Work with consumers to assist them with building skills to promote recovery and self-sufficiency
  • Involve family and other natural supports in the consumer's treatment; provide psycho-education as needed
  • Provide formal individual and group recovery support utilizing Illness Management and Recovery (IMR) and Wellness Recovery Action Planning (WRAP)
  • Provide informal wellness management and recovery strategies, including, but not limited to recovery strategies, stress management, psycho-education, symptom management, and social skills
  • Provide at least 20 minutes of formalized education to team members each six months in recovery principles and strategies, including topics such as social support, relapse prevention, and self-determination
  • Provide informal consultation with other team members
  • As needed assist with consumer income support by escorting consumers to appointments to apply and recertify health insurance (Medicaid, Medicare, private insurance) and income (food stamps, SSI/D, etc)
  • Link consumers to peer support and  self-help recovery resources
  • Meet monthly required level of treatment contacts
  • Complete assessments, progress notes, and significant data in required time periods
  • Provide culturally competent services that are individualized to client needs and reflect the integration of their race, color, religious creed, disability,  handicap, ancestry, sexual orientation, gender, gender expression, national origin, age and sex
  • Maintain confidentiality and adhere to HIPAA
  • Any other duties as assigned
  • High School Diploma/GED plus three years of relevant work experience required
  • Must self-identify as an individual with an addiction who is currently or formerly a recipient of addiction services.  As a peer in recovery from addiction and/or co-occurring serious mental illness and substance abuse the peer must be willing to utilize recovery experience to engage and support consumers;
  • Strong knowledge of wellness, recovery and self-help
  • Ability to work effectively with diverse team and remain highly organized and self-motivated
  • Proficient in computers
  • Ability to pass a drug screen and background check
  • Valid driver's license and car insurance
