A licensed practical nurse (LPN) is responsible for nursing care of clients.  The LPN generally reports to the Registered Nurse, or Director of Nursing Services, or Supervising Nurse, or the Division Director.


  • Under direct supervision, provide general nursing care to clients at the facility:
    • Take and record client's vital signs
    • Administer prescribed/specified mediation and note time and amount on client's chart
    • Observe clients and report adverse reactions to medication or treatment to supervisor
    • Inventory and requisite supplies
  • Other duties as assigned


  • High School Diploma, or equivalent;
  • Graduated from an approved school of practical Nursing; and is licensed to practice as a Practical Nurse in New York State.
