The Registered Nurse PRN will coordinate client care with the interdisciplinary team including physician, client/family and referring agency; and administer skilled nursing care for clients of all ages in their place of residence and assumes the responsibility for coordination of care.

• Case manages and provides clinical directions to the interdisciplinary team, physician and family
• Implement/develop/document the plan of care
• Provide care utilizing infection control measures that protect both the staff and the client (OSHA)
• Assess home health clients to identify the physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations
• Assure continuity of quality client care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with requirements of third party payor.
• Perform on-call responsibilities and provide on-call service to clients/families as assigned
• Other duties as assigned

• Graduate of an accredited RN School of Nursing
• Current State license as a Registered Nurse, current State Drivers License.
• Minimum two years experience as Registered Nurse in a clinical care setting, home health preferred.
• Proof of current CPR, negative TB screen and Hepatitis profile.
• Reliable transportation and valid auto liability insurance.
• The employee must occasionally lift and/or move up to 50 pounds.
