Job Description:

In collaboration with the multidisciplinary team, the Nurse Navigator is responsible for the supporting the patient throughout the continuum of care, functioning as a patient advocate, liaison, and educator.


Minimum Requirements


  • Bachelors of Science in Nursing


  • Minimum of two [2] years clinical nursing experience
  • Orthopaedic and/or Trauma Nursing experience

Licensure, Certification, Registration

  • Active CT Registered Nurse License

Preferred Requirements


  • Master’s Degree in Nursing or related field, preferred

Licensure, Certification, Registration

  • Certification in Specialty
