SUMMARY: The Patient Services Coordinator (PSC) is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The Patient Services Coordinator also assists the Clinical Field Staff Supervisor with day-to-day office and staff management as directed by the Branch Director. The PSC may be licensed or non-licensed.



  1. Manages schedules for all patients. Edits schedule timely when patient visits must be reassigned.

  2. Receives second calls from the field staff during the Clinical Field Staff Supervisor's lunch hour, weekly Case Conference, and any other busy time as directed by the Branch Director. Refers clinical questions to Branch Director as necessary.

  3. Maintains the on-call notebook according to the On-Call procedure.

  4. Maintains the Client Hospitalization Log, including entering coordination notes twice weekly into Homecare Homebase (HCHB), and sends electronic logs to Account Executive(s).

  5. Completes requested schedule as task appears on the action screen according to Branch Scheduling Guidelines and Caregiver Optimization standards. Ensures licensed nursing staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction with the injection visit.

  6. Completes requested schedules for all add-ons and applicable orders:

    1. Schedules discharge visits/ OASIS Collection or Recert visits following case conference when task appears on action screen.

    2. Schedules TIF OASIS Collection visits and deletes remaining schedule.

    3. Follows up with clinician(s) on missed visits to ensure the visit is truly missed or if it can be rescheduled.

    4. Reschedules declined or missed (if appropriate) visits.

    5. Processes reassigned and rescheduled visits.

    6. Ensures Supervisory Visits are scheduled according to branch protocol and regulatory mandates.

    7. Ensures Therapy Reassessment visits are completed timely.

  7. If licensed, and as directed, may receive lab reports and assesses for normality. Faxes copy of lab to physician, makes a copy for the Case Manager and routes to the Medical Records Department as directed. Initiates Employee/Patient Infection Reports as directed.

  8. Prepares weekly clinician schedules. Performs initial review of weekly schedule for productivity/geographic issues and forwards to clinician(s).

  9. Sends schedule to Branch Director for approval prior to distribution to staff if productivity is not being met.

  10. Verifies paper notes in scheduling console as needed.

  11. Assists with internal transfer of patients between branch offices.

  12. Reviews Coordination note section and Administrative task section daily and throughout the day to help maintain communication with clinicians.

  13. If licensed, may be required to perform patient visits.

  14. If licensed, may be required to participate in on-call rotation.

  15. Reads and adheres to and participates in the Branch's mandatory HIPAA / Privacy Program and Employee Compliance Program.

  16. Reads and adheres to all Branch policies and procedures and follows Employee Handbook guidelines.

17.  Completes all other duties as assigned.


  1. If licensed, must be a graduate of an accredited school of nursing.

  2. If licensed, must be licensed as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state licensed to practice. Registered Nurse (RN) preferred.

  3. One year of clinical experience in a hospital, or nursing home setting required.

  4. One year of home health experience in the past 24 months and a good understanding of staffing and scheduling requirements related to home health is required. 

  5. Must be organized and able to manage multiple tasks simultaneously.

  6. Must have exceptional customer service skills and be able to communicate well with referral sources, patients, family members, and branch employees.

  7. Must have/maintain a valid driver's license and auto liability insurance.

  8. Must be flexible with work schedule.
