Description:

The Case Associate works directly with the residents of the Abraham Residence III formulating and implementing the resident’s treatment plan as determined by the department's clinical staff. Under direct supervision, job duties for the position may include but are not limited to:

Job Responsibilities

 CLINCAL

  • Routine communication with community contacts on behalf of residents
  • Identifies and makes use of appropriate community resources for residents
  • May interpret agency function to client and demonstrate this in terms of concrete help
  • Provides structure to the client, evaluates and assess usage of independent living skills and provides guidance in these areas to the client.
  • May run groups, under the supervision of the social worker
  • May participate in intake, assessment, discharge and follow-up treatment conferences.

ADMINISTRATIVE

  • Coordination of Continuous Performance Improvement Meetings
  • Coordination and tracking of Incident Review and Reporting
  • Monthly OSHA walk through
  • Coordination of intakes and apartment unit availability
  • Monthly Fire Drills
  • Group Coordination
  • Special events planning (holiday celebrations)
  • Monthly LS-3 CUCS Census reporting

REQUIREMENTS:     

An individual with a BA Degree with some experience in a community or social service group, or at least five years experience as a Case Assistant in a community or social service group, etc.
