Job Description
OVERVIEW: The Case Worker works directly with the entire family referred to the preventive services program by providing a range of services, both directly and through referral that ensure safety and stability for the children.
RESPONSIBILITIES include but not limited to:

Provide a range of concrete and clinical services and intervention to ameliorate the reasons family became known to the preventive services program.

During the intake and assessment phase, introduce Family Team Conferencing (FTC) model to the family.

Using both ACS and JBFCS tolls, assesses and reassess the individual members of the family in areas of safety and risk, mental health, physical health, substance abuse, domestic violence, housing and employment needs.

Locate and ensure appropriate community resources for client; communicate with community providers ongoing and request their participation in FTC.

Develop service plans in concert with the family as well as safety interventions.

Provide clinically focused counseling to all family members.Identify and provide access to specific appropriate concrete services, when necessary or requested, and are in the best interest of the client.

Maintain frequent face to face contact, meeting at least minimum standards of home based and office visits including observing all of the children monthly.

May run groups for parent or for youth.

Maintain the case records consistent with JBFCS and ACS standards.

Other duties as assigned.
REQUIREMENTS: LMSW preferred, MSW or BA Degree with some relevant experience in child welfare or family counseling services.
