Essential Functions of Activities Assistant/Transportation Driver:

  • Schedule, pre-plan and implement floor activities based on facility activity schedule
  • Use Monthly calendar ideas to plan and organize “special” floor activities
  • Prepare special snacks with residents
  • Set-up the dining room tables for activities
  • Clean-up after all activities
  • Complete required logs and charting
  • Keeping all activity areas clean, safe, and free from clutter
  • Assist residents to and from common areas as stated by their care plan
  • Assist other departments with residents as needed and required
  • Weekends, holidays, and evenings required
  • Transports residents, staff, volunteers, and others to scheduled events.
  • Coordinates transportation services with appropriate staff members
  • Assists residents in and out of the vehicle, using proper safety procedures
  • Operates mechanical lift as required for wheelchair bound patients
  • Follows written and verbal instructions before leaving facility with residents
  • Obtain appropriate paperwork before leaving facility with resident
  • Assess and correct inappropriate resident appearance before leaving facility
  • Follow Resident’s Rights
  • Follow facility safety protocol
  • Dress professionally and represent the facility in a professional manner
  • Monitors and reports necessary mechanical maintenance needed to ensure safe operations of vehicle, including reporting of any mechanical problems encountered with vehicles
  • Inspects and maintains vehicle supplies and equipment, such as gas, oil, water, tires, lights, and brakes, to ensure that vehicles are in proper working conditions
  • Maintains records such as vehicle logs and destinations as required
  • Reports any issues or problems that may arise to the Transportation Coordinator and Administrator
  • Complies with state, federal and all other applicable health care and safety standards
  • Performs other duties as assigned

Requirements of Activities Assistant/Transportation Driver:

  • Minimum Education: High School Diploma or HSD equivalency is required
  • 6 college credits in applicable study as required by NCCAP is preferred
  • 500 hours of experience in activity programming is preferred
  • Minimum of 1 year of commercial driving experience preferred
  • Certification as Occupational Therapy Assistant or NCCAP certification is preferred
  • Must be CPR certified
  • Valid State Insurance and License
