The Certified Nursing Assistant (CNA) provides services to Hope support recipients by providing direct care and services under the direct supervision of a Lead or Home Alliance Coordinator and the oversight of a Registered Nurse. The CNA teaches activities of daily living, as well as social and communication skills to foster social independence. The CNA uses their training to perform health related tasks and to observe the health status of support recipients and assist them in following health care directives received from medical providers. This position provides opportunities for the support recipient to work on goals and objectives as outlined in the Plan of Care and/or support plan. The Certified Nursing Assistant maintains loyalty to the organization, its beliefs, values, mission, vision and expectations and to the recipients of services.
Essential Job Duties
1. Provides support services as outlined in the Plan of Care and/or support plan and any health care protocols, including, but not limited to, activities of daily living, activities in the community, and health care or medical supports.
2. Obtains and records vital signs such as pulse, respiration, blood pressure, and temperature, record fluid intake and outputs, weights, and other observations.
3. Provides daily care to individuals, such as providing assistance with meals, baths, and treatments. Assists recipients with self-administration of medication as directed.
4. Collaborates and communicates with all members of the team to assist with the coordination of services, development and implementation of support plans, and monitors their effectiveness.
5. Observes and documents recipient’s health, physical condition, and wellness status on a daily basis and reports changes or concerns to their direct supervisor and the Health Services Supervisor. Keeps parent(s) and or guardian(s) informed as requested.
6. Models and mentors positive communications and behaviors. Pre-teaches alternatives and manages less desirable behaviors through informal assessment and implementation of strategies.
7. Demonstrates sensitivity to the emotional, social, and mental health needs of each individual through skillful and directed interactions.
8. Attends health care appointments with recipient to ensure providers have current and accurate health and safety information and to assist and advocate for the support recipient as needed.
9. Coordinates transportation for support recipients, which may include transporting in personal vehicle, arranging public transportation, and/or through the use of vehicles assigned to the Agency’s Assisted Living homes.
10. Develops and maintains positive professional relationships with support recipients, families, guardians, co-workers, medical providers and community members.
11. Completes accurate, timely, and thorough documentation, including but not limited to, service notes, goal tracking, incident reports, and timecards, to be in compliance with Agency standards and federal and state regulations.
12. Works a variety of hours and locations and with a number of different support recipients.
13. Teaches and assists support recipients to safely access services and participate in community activities.
Education and Experience
1. High school diploma or GED required.
2. One year of Certified Nursing Assistant or related experience required. Experience may include non-paid experience in a health related capacity.
3. Current unencumbered state or nationally recognized certification to practice as a Certified Nursing Assistant (CNA).
1. Possesses a valid Alaska driver’s license and has at least one year of driving experience (may include permit time). Obtains a driving record at time of employment and as requested by management.
2. Ability to safely operate Agency vehicles when needed. Possesses reliable transportation and vehicle insurance when transporting support recipients or conducting Agency business.
3. Meets State and Agency requirements for passing a criminal background check upon hire and as required thereafter.
4. Lifts 35 pounds from a squat and kneel position on the floor.
5. May be required to frequently walk, bend, stoop, kneel, reach, pull, and/or push.
6. Completes all required State, Agency, department, and position specific training as assigned.
7. Obtains a tuberculin clearance at time of employment and annually thereafter.
