Community Outreach Case Manager
Overview
Since 1977, Sojourner Center has been a safe haven from domestic violence for women and their children in Arizona. Since our founding, we have transformed lives by providing shelter and support services to more than 60,000 adults and children affected by domestic violence. We provide a broad spectrum of services to victims of domestic violence, while working with professionals in the field on education, research and advocacy with the goal of ending domestic violence.
Job Description:
Provides safety, support, and resources for individuals and families through case management within the framework of a variety programs and a focus on addressing trauma related to domestic violence. Engages with participants in one-on-one case management sessions, assisting community outreach participants in identifying and addressing individualized needs, offering resources and education.
Schedule: Mon-Fri, 8:00am-5:00pm (May be required to work evenings and/or weekends as necessary.)
Rate of Pay: $18.00/hour ($18.90/hour after bilingual assessment)
Responsibilities
Essential Functions:
1. Provides ongoing modeling, support, referrals, and resources—taking crisis calls and coordinating services.
2. Provides family support within the scope of the cultures of the families served.
3. Assesses full range of participants’ needs, providing follow-up while building rapport and a positive sense of community through case management and community outreach.
4. Identifies/adapts to crisis events, taking appropriate actions.
5. Determine needs of community participants and provides individualized resources.
6. Documents services in accordance with established guidelines and consistent with QA standards.
7. Facilitates classes offered on-site and in the community. Attends and speaks at community events to provide domestic violence education and awareness.
8. Provides case management duties such as; meets regularly with participants to administer ongoing assessments, completion of goal work, and supports with providing a connection to identified resources, and other duties as assigned.
9. Travels as required for home visits and community events and submits required mileage reimbursement paperwork on time.
10. Recognizes and effectively responds to impacts of domestic violence and other trauma utilizing appropriate assessments and evidence-based practices.
11. Provides ongoing resource and referral case management, specializing in transportation, pre-employment, housing, and education.
12. Engages participants in the collaborative process of identifying, planning, accessing, and advocating for resources, supports, and services to address the impacts of trauma experienced.
13. Forms relationships with staff and participants, maintaining professional boundaries, while strengthening the developmental problem-solving and coping capacities of residents.
14. Utilizing a social-ecological framework and working collaboratively as a team, links participants with systems that provide supportive resources, services and opportunities.
15. Provides support within the scope of the cultures of the individuals and families served, which may include coordination and collaboration with the Children & Youth Services Program, Lay Legal Program, Residential Programs, Pet Shelter, Human Trafficking Program, and Transitional Housing Program.
16. Provides clear, concise, and consistent documentation in accordance with established guidelines and aligned with Quality Assurance policies and procedures
17. Provides life skills training by facilitating classes and groups.
Qualifications
Education, Experience, and Other Requirements:
- Bachelor’s degree in a related field plus two (2) years of related experience; or Bachelor’s degree in an unrelated field plus four (4) years of related experience; or a High School Diploma plus six (6) years of experience in a related field.
- Prefer experience in family violence programs.
- Ability to obtain a Level I Fingerprint Clearance Card, CPR and First Aid certification.
- Valid Arizona Driver License, proof of automobile insurance and Motor Vehicle Record acceptable to Sojourner Center’s insurance company.
- Bilingual Spanish-English preferred.
