RN Affinity Cardiovascular Specialists (ACG)
Under general supervision, provides patient education, support, interventions, care coordination, health promotion and prevention to various cardiac populations. Will use a process of enrollment, assessment, planning, and care in accordance with physician orders, the established plan of care, the patient's diagnosis, policies and procedures, and the Nurse Practice. The nurse will take a health coach approach to engage with and support patients in shared decision of behavior modification, achieving their goals and improving quality of life using the Mount Lasting Change Model, Motivational Interviewing, Fogg Model, and the Transtheoretical Model of Behavior Change. Associate's degree in Nursing and currently licensed as an RN in the State of Alabama required. Must have Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) through either AHA or American Red Cross. Critical care or home health experience is preferred.
* Helps to identify patients who are appropriate for RPM.
* Establishes vital sign parameters, obtains physician orders, may develop a plan of care, and documents findings.
* Develops and maintains rapport with patients and caregivers via phone and/or video communication.
* Assesses and evaluates patient data and needs; providing appropriate support. IE: Patient/family health education, direction, care coordination among disparate providers and intervention, and health coaching.
* Notifies physician and other care agencies of changes in patient condition or status, sends reports, obtains new orders, and may modify the care plan as needed.
* Identifies the need for and recommends appropriate disciplines and community resources.
* Informs team lead of perceived problem areas related to program, patient care, or community relations.
* Demonstrates a professional attitude, initiative in self learning, sound judgment and continuity in interactions with physicians, other health care providers, and community agencies and the patient and family.
* Acquires and utilizes knowledge of community resources.
* Demonstrated clinical, problem solving, critical thinking, conflict resolution, interpersonal and communication skills.
* Tech-savvy and experienced using digital platforms while demonstrating flexibility and adaptably to change.
* Critical care, home health, remote patient monitoring, or health coaching experience preferred.
