POSITION PURPOSE

The Associate Director of Health Outcomes for Healthcare Home is part of the person-centered integrated care team and is part of the Healthcare Home Leadership Team responsible for health outcomes of individuals enrolled in Compass Health’s Behavioral Health Healthcare Home level of service.

ESSENTIAL FUNCTIONS – JOB SPECIFIC

• Client Education
o In consultation with HCH Leadership, including nursing and medical leadership, develop educational pieces for clients regarding chronic disease self-management/disease prevention in an effort to improve health outcomes
 Ensure that educational pieces are at the appropriate literacy level for clients and are as visually appealing as possible
 Ensure that educational pieces are “non-prescriptive” and “client friendly”
• Assist the HCH Dietitian in preparing client educational pieces
• Staff Education
o In consultation with HCH Leadership, including nursing and medical leadership, develop educational pieces/trainings for CPRC/SUD staff regarding chronic diseases/disease prevention in an effort to improve health outcomes
 Ensure that educational pieces/trainings are at the appropriate literacy level for non-medical staff
 Ensure that educational pieces/trainings contain stage-matched interventions so that they can be used with clients according to their readiness for change
 Maintain the Relias training regarding HCH for new IHSs during onboarding
 Hold face-to-face/virtual meetings with new IHSs and CPRC/SUD Directors and Supervisors as needed
 Develop and deliver trainings for CPRC/SUD staff on topics identified by the Leadership Team or by request of the CPRC/SUD staff
 In consultation with the HCH Leadership Team and Medical Leadership, develop monthly education pieces to be sent to CPRC/SUD staff regarding the identified monthly Disease Management “flags”
• In consultation with Regional Nurse Directors and Medical Leadership, coordinate PCPC lunch n learns
• Assist in coordination of PCPC education to HCH NCMs regarding best practices in chronic disease treatment and prevention
• PCP Usage
o Assist the Quality team in running regular reports regarding PCP usage and relay to NCMs and CPRC/SUD staff in an effort to increase PCP usage
o Assist the Quality team in maintaining and promoting usage of the Medical Provider Form in Avatar
o Maintain PCPC schedule and communicate any changes/time off requests to Nurse Directors
• Population Health
o Assist HCH Director of Quality in developing and operationalizing process improvement initiatives
related to Healthcare Home Disease Management Indicators
o In coordination with Chief Medical Director and HCH leadership, develop care paths and data initiatives around Healthcare Home population health outcomes
• Keep abreast of current Chronic Disease Self-Management/Disease Prevention materials – including consultation with HCH Nursing and Medical Leadership, consulting respected websites such as the CDC, NIH, ADA, AHA, etc.
• Maintain the Healthcare Home Educational Folder – keeping educational materials up to date
• Assist the Healthcare Home Quality Director with tasks related to Quality/Compliance as needed

QUALIFICATION(S): EDUCATION

• Bachelor’s degree in public health, human services, or a health-related field required
• Master’s degree in public health preferred

QUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS

At least two (2) years experience preferred
