The Commons in Lincoln is seeking a Resident Care Coordinator to join our campus. The Resident Care Coordinator will assist the Resident Care Director and Mind & Memory Care Director with administrative tasks for our community. Candidates must be a licensed CNA with previous experience in scheduling and excellent customer service skills. This position follows a leadership staffing model that consists of routinely working a flexible schedule, primarily Sunday through Thursday.



PRINCIPLE DUTIES AND RESPONSIBILITIES

  • Shows a consistent ability to appropriately set priorities and manage time and workload
  • Shows a consistent ability to coordinate multiple tasks
  • Demonstrates problem solving, decision making skills, and works well under pressure
  • Assists with accurately assessing, communicating, and documenting residents' status
  • Assists with effectively handling follow-up issues with residents and families
  • Successfully implements the plan of care
  • Assists Resident Care Director when reviewing service action plan changes with families
  • Schedules resident care associates
  • Supervises all activities of the resident care associates

EDUCATION/EXPERIENCE/LICENSURE-CERTIFICATION

  • Certified Nursing Assistant in MA with previous experience in scheduling
  • Computer skills preferred
  • Represents the community with a positive attitude and pride when interacting with potential residents and families
  • Physically able to move at least 50 lbs. without assistance
