Job Summary:

The Providence Center Case Manager Young Adult Health Home (YAHH) will provide case management services for young adults aged 18-25 with mental health and/or co-occurring disorders and their families in the greater Providence area. Will assess, plan, develop, coordinate, and provide treatment and supportive services to young adults with mental health issues.

Must meet Health Home diagnosis criteria.

Provide psychiatry, therapy, case management, nursing, and substance use counseling.

Duties and Responsibilities:

  1. Coordinate a variety of services.
  2. Provide support counseling, problem solving, contracts, and limit setting.
  3. Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, including working with substance abusers, as it relates to case management.
  4. Make assessments and provide appropriate crisis interventions and risk management as needed.
  5. Teach consumers adult daily living skills.
  6. Maintain a clean, healthy, and safe environment at all times.
  7. Initiate treatment and advocate for consumer within the network of care.
  8. Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
  9. Transport consumers in own vehicle or Center van to planned activities and appointments.
  10. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements within required time frames and abide by Center confidentiality policy.
  11. Maintain cooperative relationship with Center staff, consumers, community agencies, and the public.

Requirements:

Bachelor's degree from a four-year accredited college or university in Human Services preferred. Associate's degree required.

Experience in Human Services Field preferred.

Must have valid driver's license and insured auto.
