MDS Coordinator

The primary responsibility is to assess residents, evaluate and maintain MDS's for timely, accurate completion and submission. - Assess residents in accurately complete the Minimum Data Set and Resident Assessment Protocol
- Coordinate MDS and Care Plan Team
- Implement and monitor individual care plans for residents based on MDS Guidelines
- Maintain a schedule of due dates for MDS and Care Plans including new admissions and those with significant changes
- Submit the MDS to the appropriate agency
- Must hold a current, unencumbered, Active Illinois RN / LPN License and CPR
- 1+ year Experience in LTC as an MDS Coordinator - preferred
- Ability to organize and prioritize
- Make sound nursing decisions within scope of practice
- Strong written and verbal communication, along with critical thinking abilities
- Solid understanding of current state, federal and local standards, regulations and guidelines that govern our facility
- Possess leadership abilities
