Job Description

Essential Functions

  • Oversees Release of Information process

  • Assembly of medical records

  • Analysis of medical records

  • Assist physicians with deficiencies

  • Send patient records to next level of care

  • Track location of records

  • Performs all “other\u0022 duties as assigned

  • Audit charts for appropriate documentation        

  • Distribute reports to units

  • Files patient documentation into medical records

  • Copies charts for other departments

  • Maintain documentation in HIPAA Log referencing all information distributed.

  • Develops and maintains systems to monitor departmental processes.

  • Coordinates in obtaining laboratory testing that was completed by each patient.

  • Assists in charging out the various laboratory tests on the individual Medication Administration Record (MAR.)

  • Ensures that the completed laboratory testing is taken to the Director of Nursing (DON) for further review.

  • Completes and remits Short Term Disability papers and FMLA papers for patients who provided the forms.

  • Follows through on requests and correspondence for counselors, physicians, Chief Medical Officer, Clinical Supervisor and at times the patient.

  • Maintains facility administrative reports.

  • Maintains the policies and procedures for Medical Records.

  • Coordinates with all departments and staff members to maintain patient confidentiality.

  • Audits charts for completeness and when complete closes the patient’s record and files the record away.

  • Completes outside agencies request for patient information and records in HIPAA log.

  • Maintains patient lists, patient assignments and checklists on a daily and biweekly for the inpatient facility.

  • Coordinates copying of charts for the outpatient services at McCallum Place.

    Additional Duties and Responsibilities

    Complete other duties as assigned by Supervisor

Education and Experience Requirement

  • High school graduate with college or technical school preferred.

  • Two (2) to Three (3) years’ experience in the management of medical records.

  • ART or RRA preferred, but not required.

  • CPI (Crisis Prevention/Intervention)

  • Specialized training or knowledge in the management of medical records, knowledge of medical terminology. Ability to accurately transcribe dictation, word processing and computer use.

  • Annual Tuberculosis test required, initial screening due within 14 days of hire.
