Daily Responsibilities: 

The RN Case Manager will be responsible for: Assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care.

Requirements:

• RN License in the state of California
• 2+ years of Case Management experience from managed care (preferred), home health, or hospital setting
• 1+ years of Med-Surg experience

Hours for this Position:

Monday – Friday, from 8:00am to 5:00pm
