                                           POSITION DESCRIPTION

Position Title: Mental Health Worker (ALL SHIFTS, WEEKENDS ONLY)

Department: Nursing Services

Reports To: Charge Nurse / Nurse Manager / Chief Nursing Officer

Supervises: N/A


The Mental Health Worker is responsible for providing direct patient care to Children, Adolescents, Adults and Geriatrics diagnosed with psychiatric and chemical dependency disorders; to ensure the safety and well-being of these patients; to support the maintenance of therapeutic milieu. Responsible for maintaining a safe, therapeutic environment, reporting concerns to the nurse, documentation, facilitating or assisting with groups when necessary, following program specifications, and risk management. Responsible for providing direct care and performing duties in accordance with hospital policies and procedures in collaboration with the nursing services team.


High school level reading and writing skills; CNA Certification preferred; acceptable oral and written communication skills; legible handwriting; ability to maintain information as highly confidential; telephone etiquette and customer service skills by phone and face-to-face; familiarity with medical and psychological terminology preferred; skills in dealing courteously with the public and other staff within the organization; basic knowledge of human behavior or principles of psychology useful; skills in prioritizing and in managing time; problem-solving skills; use of appropriate social skills, including demonstration and respect for staff and patients; empathy toward patients; basic knowledge of 12-step treatment program; insight regarding denial and resistance to treatment; knowledge of the recovery process, including relapse and relapse prevention. Must be currently CPR certified through the American Heart Association only.

  1. Adheres to facility standards concerning conduct, dress, attendance, punctuality, and meal & rest breaks.

  4. Maintains proper boundaries when interacting with patients and their families, visitors, professional staff, vendors, and co-workers.

  12. Maintains only appropriate interpersonal communication with other staff members, patients, visitors, and clinicians

    Performance Improvement/Education

  16. Completes competency tests, programs, and self-assessments as required.

  17. Attends all mandatory facility in services, department meetings, training and staff development activities as required.

  18. Maintains and forwards up-to-date documentation of certifications, registrations and licenses to Human Resources for placement in employment records.

    Safety Rules and Procedures/Infection Control

  19. Complies with safety and security policies and procedures, including reporting of incidents and injuries and responding to codes.

  20. Keeps workspace clean and free of work hazards, follows hospital hazard communication program. Maintains equipment in safe working order, reports problems to supervisor and/or maintenance.

  21. Complies with hospital policy regarding identification badge.

  22. Consistently utilizes universal / standard precautions and environmental safety guidelines.

  23. Assembles and disassembles charts as assigned, maintaining correct chart order. Files chart forms correctly. Checks admission consents for completion as assigned.

  24. Demonstrates competency to complete the MHW section of the Nursing assessment.

  25. Is knowledgeable of nursing department and hospital policies and procedures. Takes initiative to locate and clarify when unsure of specific policy.

  26. Documents patient care and observations utilizing the Master Treatment Plan as a guide for care rendered. Assists with documenting treatment plan problems as directed by Charge Nurse. Documents all procedures and pertinent data in patient records.

  27. Checks Nursing Care Plan and Treatment Plan on assigned patients daily and follows plan of care which may include special care, monitoring of vital signs, heights, weights, meals, special observance levels and specimen collection. Reports significant findings to the Charge Nurse.

  28. Demonstrates maximum utilization of skills to teach, direct, and assist patients when providing patient care.

  29. Initiates and documents patient/family teaching on the Interdisciplinary Teaching Flow Sheet.

  30. Documents on the Daily Nursing Flow sheets and any unusual occurrences in the Patient Progress Notes.

  31. Takes report at the start of shift and reviews assignments with Charge Nurse to ensure clear understanding of specific shift responsibilities.

  32. Conducts a head count at the start of each shift and checks against rounds sheet and patient board.

  33. Employs principles of communication, interviewing techniques, and problem solving and crisis intervention when performing psychotherapeutic interventions.

    • Recognizing cues to behaviors.

    • Using less restrictive interventions.

    • Acts as a patient advocate and assure the patient’s rights are upheld.

    • Employ hospital approved management of assaultive behavior techniques (CPI) as necessary.

    • Use of seclusion.

    • Use of restraint.

    • Utilizes therapeutic communication techniques in:

      • Communicating with patients and families.

      • When leading Nursing Groups.

    • Able to conduct Nursing Goals/Wrap groups as directed and documents on the Teaching Flow Sheet/Nursing Flow Sheet.

    • Responds, remains calm and takes direction in all emergency situations. Understands Hospital codes and responds appropriately to ensure the safety of patients, staff, and visitors.

    • Accountable for patient safety, check patients through rounds and compliance with suicide or elopement precautions, and performs room checks.

    • Checks patients for contraband and manages the patient’s valuables and belongings.

    • Assists patients through the discharge process.

    • Completes ongoing checks of Unit environment for safety and therapeutic atmosphere, reports significant issues to Charge Nurse.

      • All doors to restricted areas are empty and locked.

      • Seclusion and time out areas are secure, clean and ready for use.

      • Patient room checks.

    • Demonstrates competency in checking patient vital signs.

      • Logs vitals as they are taken

      • Vital signs are taken on all admits.

    • Assists patient’s as needed with personal hygiene, meals, or other specific needs to ensure the patient’s safety, comfort or welfare.

    • Plans and organizes shift duties to meet changing needs of the Unit. Recognizes and responds to priorities in patient care responsibilities.

    • Utilizes reality orientation, redirection, and limit setting techniques to facilitate milieu management and functioning.

      Age-Specific Care

    • Has the knowledge of growth and development for patients of all ages. Has demonstrated competency in planning and providing care based on a patient’s age-related / developmental needs.

    • Ensures patient room assignments are appropriate to patients’ needs, age, gender, level of functioning, and condition.


Reaching: Extending hand(s) and arm(s) in any direction.

Walking: Moving about on foot to accomplish tasks, particularly for long distances.

Fingering: Picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand or arm as in handling.

Grasping: Applying pressure to an object with the fingers and palm.

Talking:  Expressing or exchanging ideas by means of the spoken word. Those activities in which they must convey detailed or important spoken instructions to other workers accurately, loudly, or quickly.

Hearing: Perceiving the nature of sounds with no less than a 40db loss @ Hz, 1,000 Hz and 2,000 Hz with or without correction. Ability to receive detailed information through oral communication, and to make fine discriminations in sound, such as when making fine adjustments on machined parts.

Repetitive motions: Substantial movements (motions) of the wrist, hands, and/or fingers.

Non-Sedentary Work:  Exerting up to 100 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. May be exposed to second hand cigarette smoke resulting from supervision of patients while they are smoking.

Ability to exercise self-control in potentially volatile situations such as being verbally or physically confronted in a threatening or aggressive manner; must be able to work and concentrate amidst distractions such as noise, conversation and foot traffic; ability to handle interruptions often and be able to move from one task to another; must be flexible and not easily frustrated in dealing with differences of opinions.
