Flexible Schedule: 15-25 hours weekly

Responsibilities:

  • Perform nursing services for assigned individuals within budgeted hours, seeking approval for any necessary deviations from the Senior Director
  • Communicate with program managers regarding policy, procedure, and program staff's ability to meet medical needs of the division.
  • Provide medication administration and health care training to program staff as requested.
  • Complete progress notes and summary of findings after each nursing visit, ensuring accurate documentation in medical logs.
  • Offer recommendations for inclusion in Service Plan Review Reports and annual Support Plan Addendum.
  • Be available for emergencies and consultation by phone, establishing arrangements with each assigned program.
  • Maintain ongoing communication with program managers, keeping them informed of individuals' issues and requesting follow-up as needed.
  • Provide consultation on nursing procedures, individuals' health issues, and individuals' admission and demission as requested.
  • Consult on doctor's orders and submit proposals for changes or implementation of policies and procedures to the program manager.
  • Ensure compliance with state health regulations, programming regulations, and organizational policies and procedures.
  • Conduct nursing visits to assigned residential homes as prescribed.
  • Perform visual checks/exams of individuals' supported, review medical logs and medication administration sheets, and provide written medical recommendations to program managers.
  • Conduct thorough physical checks of ongoing medical concerns.
  • Obtain and record blood pressure readings as per physician instructions and Support Plan.
  • Perform DISCUS and MOSES ratings for when applicable as needed.
  • Provide support to program managers in communicating medication concerns to medical professionals.
  • Perform other duties as assigned by the Senior Director.


Qualifications:

  • Licensed Registered Nurse with a current license in the state of Minnesota.
  • Demonstrated competency in the completion of job responsibilities.
  • Background clearance for licensing agencies.
  • Valid Driver's License with a clean driving record.
  • Current First Aid and CPR Certificate as required.
  • Strong interpersonal and communication skills.
  • Previous experience working with individuals with developmental disabilities preferred.
  • Multi-faceted nursing background and supervisory experience desirable.

Physical Requirements:

  • Ability to use controlled procedures and manual restraint if needed.
  • Physical tasks including bending, twisting, lifting, and carrying loads up to 20 pounds.
  • Providing physical support and guidance individuals supported and Thomas Allen employees during nursing assessments.
  • Ability to operate an automobile or use public transportation as required.
