Adult Case Manager
Job Title: Adult Case Manager: This position requires a minimum of a Bachelor’s degree in Psychology, Sociology, Human Services or Nursing. One year of experience working with either children or adults may be required.
Qualifications: Bachelor’s degree from an accredited school. Submit to and pass successfully a criminal record check and drug screening test.
Hours: Monday through Friday 8:00 am until 4:30 pm. Other times may be required with the job or adjusted by your supervisor.
Reporting Structure: This position reports directly to the Team Leader if located in an outpatient facility and/or Program Director if in a residential program.
Summary of Job Duties: The Adult Case Manager may only provide interventions to consumers with the Cumberland River Behavioral Health, Inc. The Adult Case Manager is a member of a multi-disciplinary team responsible for providing services to adults with chronic mental illness or substance abuse issues. The Adult Case Manager assists in helping the person gain access to medical, social, educational, and other support services.
Essential Duties and Responsibilities:
- Completes a written comprehensive needs assessment.
- Assists with the development of a treatment plan based on assessment data.
- Monitors the consumer’s progress with accessing needed services.
- Provides care based upon ethical concepts, adhering to established standards of practice.
- Ensures the accuracy, content, and completeness of client files.
- Provides services within time requirements as set forth in Agency policies and procedures.
- Provides services as documented in the client treatment plan.
- Performs advocacy activities on behalf of the consumer.
- Refrains from assuming any duty that is unrelated to and/or interferes with the responsibilities of the position.
- Responds to emergency situations in a timely fashion.
- Provides crisis assistance to the consumer and coordinates any needed service.
- Demonstrates the ability to work effectively with other team members as part of a multidisciplinary team.
- Other duties may be assigned.

- Knowledgeable of Behavioral Health Concepts and how to incorporate concepts into practical application.
Additional Responsibilities:
- Physically able to reach, push, pull, stoop, bend, stand, walk and lift up to 20 pounds.
- Possess a valid Driver’s License, maintain a safe driving record, and maintain the required minimum vehicle insurance amount for the State of Kentucky, if required for job performance.
Work Environment:

While performing the duties of this job, the employee may have to work in outside weather conditions, may be off-site from normal office location or placed at a Residential Program. The noise level in the work environment is mild to moderate.
Environmental Data and Job Hazards:
- Hours of work are structured 7.5 or 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands.
- Position may require duties to include routine or reasonable anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials.
- All staff are required to participate in yearly trainings regarding infection control and safety in the environment.
- All new staff must attend training regarding handling physical, verbal threats, acts of violence, or other escalating and potential dangerous situations.
- All staff must be certified in Cardiopulmonary Resuscitation within one month of employment and maintain the certification.
