The Patient Navigator will be responsible for conducting outbound calls and answer inbound calls for LIS/MSP enrollment, off-season survey follow-up, disenrollment investigation, provide patients with community resources, close gaps on HOS, etc. for all markets as needed.  The Patient Navigator will support a variety of strategic activities, based on Member Experience data and performance metrics throughout the OptumCare enterprise. This individual will maintain the highest standards to ensure the members have the best healthcare experience by facilitating the process every step of the way, and will complete daily activities reports.

You’ll enjoy the flexibility to telecommute* from anywhere within the U.S. as you take on some tough challenges.

***Preferred schedule is 8am-5pm PST or MST.***

Primary Responsibilities:

  • Serve as the clinical liaison with clinical performance and administrative staff as well as providing expertise solving member issues
  • Make outbound calls to assess members' current health status, as well as take inbound calls to resolve inquiries and assist members as needed. Warm transfer to other departments or services as needed
  • Take inbound calls when messages have been left and members are calling back to address any potential issues
  • Assess patient needs using established triage protocol, “concierge” style in order to coordinate appropriate level of assistance needed (pharmacy, referrals, etc.)
  • Perform follow up calls to high risk individuals/members to offer additional support
  • Submit targeted mailers, including but not limited to healthy checklists, measure/metric specific education, and other as needed, in coordination with CDOs
  • Email members to reinforce over the phone discussion, including community resources link, educational materials, referral or pharmacy information, etc.
  • Create and maintain community resources database for each market, in order to support members with Social Determinants of health barriers and needs
  • Assist with completion of patient satisfaction surveys if required
  • Manage multiple short and long-term projects
  • Performs all other related duties as assigned

Required Qualifications: 

  • High school diploma or GED
  • Current, unrestricted LVN/LPN license
  • 3+ years of clinical experience
  • 3+ years of health industry experience and/or in a large complex matrixed organization experience
  • Intermediate Excel skills 
  • Proficient Word and PowerPoint skills - Project scope documents, project presentations, etc. (intermediate skill)
  • Intermediate SharePoint skills 
  • Access to high speed internet and a home office

Preferred Qualifications:

  • Associate’s Degree or higher
  • Experience working in a medical care setting as a medical assistant patient care coordinator
  • Bilingual language proficiency (English/Spanish) 
  • Excellent verbal and written communication skills
  • Organized, detail-oriented

Colorado Residents Only: The hourly range for Colorado residents is $17.12 to $30.34.
