Essential Purpose: Ensuring the quality of the care and service delivery to our patient clients
Perform assessments and procedures utilizing your substantial specialized knowledge, judgment, and nursing skills based upon principles of psychological, biological, physical and social sciences.
Develop working relationships with discharge planners and social service workers.
Assess and review the matching of employee skills to client needs.
Visit in home clients to evaluate the quality of care being provided and review clinical notes.
Complete initial and ongoing assessments nursing care of all in home clients within specified deadlines.
Participate in ongoing interdisciplinary assessment of patients.
Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Obtain physician plans of treatment and orders, and initiate their execution.
Provide services that are ordered by the physician as indicated in the plan of care.
Counsel and educate the patient/client and family in meeting healthcare related needs.
Complete timely and accurate clinical notes including addressing the patient’s/client’s progress.
Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
Supervise paraprofessionals and licensed professional team members.
Secure equipment and supplies and coordinates placement for use in the home.
Accesses personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
- Active RN license in the state of Florida
- Min 5 years years nursing experience
- 3 years plus management/supervisory experience
- Current CPR
- Strong computer skills
- Able to communicate effectively with patients and staff
- Team player and team builder
- Organized and detail orientated
- Passion for great patient care and service!
