The critical role of the Case Manager

Essential Purpose: Ensuring the quality of the care and service delivery to our patient clients

Essential Functions:

  • Perform assessments and procedures utilizing your substantial specialized knowledge, judgment, and nursing skills based upon principles of psychological, biological, physical and social sciences.

  • Develop working relationships with discharge planners and social service workers.

  • Assess and review the matching of employee skills to client needs.

  • Visit in home clients to evaluate the quality of care being provided and review clinical notes.

  • Complete initial and ongoing assessments nursing care of all in home clients within specified deadlines.

  • Participate in ongoing interdisciplinary assessment of patients.

  • Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).

  • Obtain physician plans of treatment and orders, and initiate their execution.

  • Provide services that are ordered by the physician as indicated in the plan of care.

  • Counsel and educate the patient/client and family in meeting healthcare related needs.

  • Complete timely and accurate clinical notes including addressing the patient’s/client’s progress.

  • Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.

  • Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.

  • Supervise paraprofessionals and licensed professional team members.

  • Secure equipment and supplies and coordinates placement for use in the home.

  • Participate in:

    • Quality Assessment and Performance Improvement (QAPI) program

    • Office Emergency Preparedness program

  • Accesses personal health information (“PHI”) necessary to fulfill the above duties and responsibilities.  Access to use and ability to disclose PHI is further defined by each organization/department.

Job Requirements

  • Active RN license in the state of Florida
  • Min 5 years years nursing experience
  • 3 years plus management/supervisory experience
  • Current CPR
  • Strong computer skills
  • Able to communicate effectively with patients and staff
  • Team player and team builder
  • Organized and detail orientated
  • Passion for great patient care and service!
