The Program Supervisor, ALW Program is responsible for providing leadership and direct supervision to a group of BA and MA level Social Work and RN Nursing staff providing both ALW care management and CBAS assessments.   S/he will oversee the daily operations of client engagement, intake and enrollment and monthly care management.  The ALW program works with fragile adults and senior clients to assist their ability to maintain in the lowest level of care possible meeting health and safety needs while preventing institutionalized care.     

QUALIFICATIONS:

  • Minimum LCSW or LMFT licensed by the State of CA Board of Behavioral Sciences preferred. 
  • 2+ years case management experience and supervisory experience required.
  • Experience in community-based service delivery with older adults preferred.
  • Strong clinical and case management background in addition to training, and supervision experience.
  • Ability to provide leadership to develop a team of social workers and nursing staff providing assessments and care coordination through both one on one and group team meetings.  
  • Strong proficiency in computer skills (Microsoft suite: Word, Excel, etc.) and ability to quickly learn record/data base systems required.
  • Strong organizational, assessment, problem-solving, and time management skills
  • Excellent communication (written and verbal) and interpersonal skills, ability to articulate and professionally communicate with staff, clients/legal representatives, and community facilities.         
  • High degree of detail orientation and analytical thinking required.
  • Strong ability to work as part of a team and maintain professional boundaries.
  • Demonstrated ability to work independently, set priorities and complete projects within specified timeframes.
  • Culturally sensitive and able to work with a multicultural client/customer population.
  • Able to work under pressure and meet deadlines as well as be flexible and dependable.
  • Must be able to pass criminal background check (Live Scan fingerprinting) prior to beginning employment.
  • Valid CA driver’s license and proof of insurance and ability to travel to multiple work sites on a regular basis.

 

RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

  • Provides oversight of Care Management Social Workers to either facilitate the transition of clients from skilled nursing facility to the community or prevent skilled nursing placement by safely maintaining clients in lower level of care.  In addition, provides oversight to RN staff providing CBAS Assessments.
  • Provide oversight of activities related to enrollment, client assessment, care planning, interventions, and monitoring of caseloads to ensure client safety and program compliance.
  • Monitors and tracks client hospitalizations, transfers/discharges, incident reports and program waiting list and communicates client related changes with staff and facilities. Documents all client related changes in electronic database system.
  • Conducts monthly electronic file audits for regulatory compliance and quality assurance and ensure incident reports are saved in clients’ electronic file.  
  • Prepares weekly/monthly/quarterly reports for the program and DHCS. 
  • Client Communication:  Responds to client inquiries about program services and applications, conduct a follow up to ensure closure or transition into ALWP to clients and applicants on wait list, provide liaison communication with clients and families in the community regarding eligibility, enrollment process, and status. Ensure proper documentation of case management efforts and outcome in electronic data system.
  • Outreach with Skilled Nursing Facilities to identify new ALWP eligible clients and to ensure smooth transition of enrollment, ensure eligibility requirements are met, and provide guidance regarding clients’ transition plan.
  • Provide one on one supervision weekly to each direct staff in both ALW and CBAS programs.  Provide administrative oversight for RN staff completing CBAS assessments in a timely manner and submittal to health care provider as per contract. 
  • Professionally and accurately represent the agency and programs with external stakeholders, vendors, facilities and clients.  
  • Assists Program Director in preparing charts/client files for internal and external agency audits. 
  • Maintain accurate records for pre-enrollment outreach and intake activities in electronic record platform.   Utilize reports pulled from system to assist in timely program oversight and staff quality assurance of record keeping as per contract. 
  • Attend meetings as assigned including weekly one on one supervision, management meetings, ALWP association meetings.    
  • Perform all other duties as assigned.
