DIABETES CARE & EDUCATION SPECIALIST IV - CERTIFIED - DIABETES CENTER

Salary Range: $39.2400-$60.3400/hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.
Primary Duties and Responsibilities:
The Diabetes Care & Education Specialist utilizes Medical Nutrition Therapy (MNT) guidelines, regulatory standards, and evidence-based medicine to assess and document patient's nutritional needs based on pre-collected screening risk data by the patient’s physician, inpatient RN Diabetes Educators or RD. Implements care plans and counsels patients (and family members) utilizing standard protocols, reviews patient care plans, and may act as clinical instructor for dietetic interns. The Nutrition Care population includes adolescents and adults, women with gestational diabetes, patients with all types of diabetes (prediabetes, type 1, type 2), overweight/obese patients, and other patients with disease-specific nutrition regimens (e.g., renal disease, high cholesterol, etc).
In an outpatient setting, the Diabetes Care & Education Specialist IV, upon a referral from a physician, performs dietary counseling, conducts nutrition and dietary assessment, and develops nutritional and dietary treatment including therapeutic diets for patients with diabetes and other conditions listed above. Encounters will include 1:1 visits, telemedicine visits, as well as group classes. Participates in the continuous quality improvement (CQI) process by collecting data and comparing results to documented evidence-based medical/nutritional standards. Performs other duties as assigned.
Education and Experience:
- Bachelor's Degree with the Academy of Nutrition and Dietetics Internship Program.
- Ten years of experience as a registered clinical dietician and/or experience in a lead position.
Certifications Required:
- CDCES Certification required
- RD - Registered Dietician required
