The Manager of CV Disease Mgt. and Clinical Quality is responsible for all clinical quality in the CV service line and  has direct oversight of the service line Advanced Practice Nurses, Heart Failure Clinic, Stress Testing, Cardiac Disease Mgt., including but not limited to:  Heart Failure, Chest Pain, Structural Heart, Complex Ablations, Peripheral Vascular, Pulmonary Hypertension and Anti-coagulation programs of which are based on goals within the service line.  This role will also be responsible for Data Registries and Quality scorecards for the CV Service Line.  The position collaborates with appropriate members of the healthcare team to determine the appropriate and evidence-based best practice for cardiovascular disease mgt. Monitors all outcomes of CV disease mgt. to ensure that we adhere to evidence based care models and reports updates to key stakeholders and leadership.  Works with the Advanced Practice Providers and Physicians daily, overseeing care plans for patients with CV diseases while inpatient and assuring that appropriate referrals and resources are aligned for transition out of acute care.  Utilizes appropriate clinical competency to determine CV disease program f/u and continuity of care in collaboration with the health care team.
  • Registered Nurse in the state of Georgia
  • APN in state of Georgia
  • Bachelor's Degree in Nursing (BSN) from an accreditated school of nursing
  • Master's Degree in Nursing (MSN) from an accreditated school of nursing preferred.
  • Four (4) or more years job related experience
  • Four (4) or more years job related experience including Clinical experience in Cardiac Cath, Critical Care or Emergency Room preferred
  • BLS and ACLS CPR card
