A Day in the life of our Dual Role RN (Registered Nurse) Patient Care - Home Health and Facilities:
You will wake up each day excited to check on your clients in their homes or at facilities and follow up on how each is doing. As an RN in this role, your duties will be to plan, organize and direct home care services. You will conduct assessments of patient care needs, develop care plans, direct care teams, counsel patients and family members, provide healthcare instructions, prepare clinical notes and communicate patient condition information to the primary physician. You may be asked to serve in an on-call capacity, and also oversee health aides, and give bi-weekly evaluations. You will be asked to teach, train and supervise in-service programs. Your assistance is required in maintaining inventory to serve the clients that you care for.
You will verify and audit records, looking for correct dosages of medicines, proper procedures in care, and proactively plan for best scenarios to keep up most optimal health for each client. You may interact with family members of clients and will need to communicate with excellent customer service and professionalism, ensuring that all their questions are answered truthfully so that they remain confident in Dependable's competence in caring for their loved one.
You will act as Case Manager for each client when assigned by Clinical Supervisor. You will be well versed in the State Nurse Practice Act, and direct care in accordance with this act. This job will be very fulfilling as you manage, confirm the diagnosis, assist in all the facets of patient care. But most importantly you will affect the lives of our clients with your positive focus as you counsel with and provide the best care possible. Your satisfaction in knowing you have performed a job well-done each day will bring you back to each clients' home with great interest, over and over, as you serve with your heart and your excellent nursing skills.
Dual Role RN (Registered Nurse) Patient Care - Home Health and Facilities:
- A graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.
- Current licensure in state and CPR certification.
- Bachelor's degree, with one (1) year of home health care experience preferred.
- Management experience not required. Responsible for supervising home health aides.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
- Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
- Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
- BILINGUAL SPEAKING PREFERRED!
