Home Care Physical Therapist, Binghamton Area

United Health Services is seeking a Home Care Physical Therapist to join our Twin Tier Home Health Team in Binghamton, NY. This is an in home, traveling Physical Therapist position; each Physical Therapist will travel throughout the 4 counties in the Lower Tier area of NY State (Broome, Tioga, Steuben, and Chemung). This is a Per Diem Position

Shift: Days Hours per week: Per Diem Salary range: $33.57 - $46.17/hour, depending on experience


Home Care Physical Therapist, Overview

The Home Care Physical Therapist ensures Physical Therapy services are administered and completed in a timely, accurate, and reliable manner. Provides physical therapy evaluations and treatments, as well as associated clinical documentation. Maintains specific service delivery standards which address commitment to service, customer satisfaction, ongoing evaluation of customer expectations, integration with quality process, communication and interaction standards, turn-around time and information processing standards, corporate compliance requirements, and physical amenities/environment of care. Home Care Physical Therapist, Key Responsibilities and Expectations

  • Comprehends, interprets, and follows medical prescriptions for Physical Therapy. Applies the accepted principles, methods, and procedures of measurement, prediction, and evaluation tests to assess patients strengths and limitations, and assists patient to achieve objective short and long term goals. Demonstrates knowledge of side effects and complications of services provided.

  • Administers and interprets clinical physical therapy services for assigned patients. Adjusts treatments according to patient's response to planned physical therapy treatment protocol.

  • Maintains accurate records of each patient's treatment, progress, and/or regression according to the Service Delivery Standard using electronic systems, as necessary. Maintains all necessary records required to meet the criteria established by reimbursement and regulatory agencies. Completes required patient record documentation within two (2) business days.

  • Establishes and maintains effective working relationships with patients, their families, and other staff personnel. Participates in patient care conferences with other professionals involved in the management of cases. Provides patient and family education, as needed.

  • Participates in planning for patient's discharge. Makes appropriate referrals. Interfaces with vendor to provide needed durable medical equipment.

  • Provides direction and supervision to Physical Therapist Assistants.

  • Maintains necessary CEUs for licensure and/or specialty specific certifications and professional organizations.

  • Has knowledge of and participates in departmental and organizational initiatives for performance improvement (PI).

  • Assists with determining competencies of peers.

  • Documents all pertinent patient information relating to the evaluation and treatment program, utilizing Ndoc in an accurate and timely manner.

  • Assigns treatment procedures and supervises the progress of the patients assisted by Physical Therapist Assistants.

    Position Qualifications

    Minimum:

  • Graduate of an accredited school of physical therapy, receiving applicable degree in physical therapy study with Bachelor's or Masters Degree or equivalent.

  • Previous experience in an approved medical facility.

  • Possession of a current license or eligibility to be licensed as a Physical Therapist in the State of New York.

  • CPR certification

    Preferred:

  • Masters or Doctorate Degree with previous physical therapy experience in a hospital based setting.
