General Statement of Job
Under direct supervision, the purpose of the position is to provide assistance to Registered and Licensed Practical Nurses in the delivery of Health Care Services. Employees in this class are responsible for performing non-licensed work to meet the hygienic, emotional, and physical needs of residents to assist them in restoring, reaching, and/or maintaining their optimal level of function. Care is performed in compliance with a written plan of care and assignment.
This position will provide one-on-one Restorative care to the residents, along with daily C N A duties. This position will work and accompany the residents to therapy services, foot care and other care that is needed for the Residents. This position is part time and will be 3 shifts one week, and 2 shifts the next week.Responsibilities
Specific Duties and Responsibilities
Reads nursing assessment of each new resident admitted to one of the restorative programs.
Provides opportunities for residents to experience optimal independence in the following areas:
- a) Feeding - including use of a safe swallow;
- b) Performing personal hygiene/personal care (i.e., combing hair, brushing teeth, denture care, washing face and hands, bowel and bladder continence, skin protection and maintaining skin integrity);
- c) Performing restorative exercises, including routine range of motion exercises;
- d) Using adaptive equipment; and using and caring for splints (i.e., application, scheduled wearing, cleaning of) as needed;
- e) Dressing self (i.e., buttoning, tying shoes);
- f) Ambulating and transferring (i.e., using a gait/transfer belt and appropriate assistive devices);
- g) Positioning in a bed, chair or wheelchair;
- h) Communicating using techniques for the hearing or speech impaired resident;
- i) Interacting socially by using validation therapy, cognitive stimulation and sensory stimulation.
Ensures that the following restorative programs and documentation are in place and monitored on a daily basis:
Restorative flow sheets;
Dining Room consumption;
Restorative range of motion exercises;
Walk to dine program;
Dining room supervision to include right diet, right person, likes, dislikes, thickened liquids, enough time to eat and pleasant dining experience.
Assists residents with identifying food arrangements (i.e, informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, removes sealed covers).
Assists with meals in the Dining Room and the Assisted Dining Room. Observes and assists with positioning of residents at the tables for meals. Ensures adaptive feeding aids are used as prescribed. Assists residents with restorative positioning to improve functioning. Informs Restorative Nurse of changes in residents eating patterns or if additional assessment may be needed.
Assists residents with splints who require splints.
Completes and records weights for all new residents.
Completes weekly, monthly and other prescribed weights and documents.
Records the resident's food/fluid intake as needed. Reports changes in the resident's eating habits to the Restorative Nurse/Nurse Manager.
Transports residents using wheelchairs, stretchers, recliners, or assists them to walk with or without assistive devices.
Assists or performs range of motion, turning, positioning, lifting, and transferring of residents.
Measures/takes vital signs, blood pressure, food and liquid intake, resident output, and related data.
Assists professional nursing staff with complex nursing care of residents.
Reports all problems when observed to the Charge Nurse on duty and/or to the Restorative Nurse or Nurse Manager.
Reports when coming on duty and/or leaving for any reason.
Follows written restorative/plan of care and assignments, including performing rehabilitative/restorative tasks such as helping residents learn how to feed themselves, ambulate, and toilet train.
Ensures resident safety during care and monitors for environmental hazards and unsafe conditions.
Records all entries on flow sheets, to include nourishments, bath lists, weight, bowel and bladder records, and intake and output records and other required notes.
Charts in an informative and descriptive manner.
Records all supplies taken for each specific resident.
Observes and reports all changes or unusual findings (physical condition, attitude, reactions, appetite) in residents’ conditions to the Staff Nurse and/or Charge Nurse.
Assists in problem solving and other restorative care issues and concerns.
Prepares residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place clothing protectors, takes to/from dining room).
Serves food trays. Ensures that resident receives diet as ordered. Assists with feeding as indicated (i.e., cutting foods, feeding, assisting in dining room supervision). Reports any problems with tray delivery (i.e., not receiving foods requested or incorrect diet)
Serves food when requested between meal and bedtime snacks.
Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, take to bathroom, brush teeth, clean dentures).
Check rooms for food articles (i.e., food in proper container, unauthorized food items).
Documents: care, treatments, and observations on flow sheets or other documents as directed. Maintains proper filing of resident documentation.
Emergency duty may be required of the incumbent that includes working in special needs or Red Cross shelters or to perform other emergency duties including, but not limited to, responses to threats or disasters, man-made or natural.
Performs other C.N.A. duties as instructed.
High school diploma or GED preferred.
Minimum 2 years as C.N.A. required.
CPR certification required; Restorative Certification preferred.
State of Florida, Department of Health Certified Nurse’s Assistant required.
